Fistula - Branching

Just wondering if anyone had suggestions or experience they could share regarding fistula branching. I'm trying to decide on going ahead with surgery.
 
My fistula had a secondary track my CRS called it an "extension". It is part of the same fistula and more superificial. It was annoying as it drained. Nevertheless I just had surgery and hopefully it will be taken care of along with the primary track.

I have multiple fistulas with branches. My GI specialist said she has never seen fistulas that run up instead of down. Of course mine have to. They are causing me to not be able to digest anything. I got so weak I collapsed and was hospitalized for a week. Have been home on TPN for 5 miserable weeks. I have been on Remicade for months and has healed part of my colon. Serious side affects of my hair falling and skin issues. I am getting ready to have surgery to try to remove the fistulas and damaged area. They said that this is my last ditch effort. Wish me luck.

I don't have Crohn's but I read a lot through these forums.
