Can CTs miss inflammation?

Can CTs miss inflammation?

I had a CT a month ago which showed significant inflammation. I have a CT enterography this past week which was normal. My GI said that CTs can miss inflammation. Anyone have this happen?

Typically you have upper /lower scope ,MRE (mri with entography) and capsule endoscopy (pill cam )
Then they get the full picture
But even that can miss some stuff

Scope /biopsy can tell the story
That’s why they do all of the tests
They need the whole picture first
My kiddo had a visually clean scope
But biopsy results showed chronic abd acute inflammation as well as non caseating granulomas
MRE showed mild thickening at ti
Pill cam showed nothing
So they do need all of the tests combined to figure things out

Thank you so much for the information. I am not ready to accept that I may have IBD. It stinks that I am stuck on prednisone until I can have the scope.

Prednisone clears inflammation
Normally they don’t give steriods until after scopes
Because otherwise your scope will be clean (no evidence of disease ) same as your cte was
We were told no steriods for 6 to 7 weeks prior to a scope for dx
Otherwise they don’t know what they fixed
And prednisone fixed inflammation everywhere for multiple types of diseases
Especially if it’s a month from
Now
Wow 😯

I will be on prednisone for the next 8 weeks. By the time of the scope I will be down to 30 mgs. No one ever mentioned that I should be off the prednisone. I will not be completely off the prednisone until the middle of August. Now I am worried that I will go through all the scope prep for nothing. I have an IBD test that came back consistent for Crohn's but that was from 7 years ago. My GI did recheck that but still waiting for those results. They put my back on the prednisone because my symptoms came back a few days after I stopped them the first time.

Unfortunately if you have been on steriods awhile
Ugh
You need to call your Gi
Steriods can’t be stopped suddenly
They need to be weaned slowly so
Definitely talk to your Gi

My GIs response it to continue the current treatment plan. I didn't know that it could affect the biopsy results. I will have to call and ask him about that. I would be kind of nervous to wean off the prednisone without having a backup medication due to the quick reoccurrence last time I went off the prednisone. Thanks for your help. I feel kind of like a fish out of water not know what to expect and everything that goes with it.

If you were dx with crohns 7 years ago what meds were you taking to keep inflammation down during that time
You may have enough evidence from previous tests and your Gi is just trying to calm things down while waiting to scope to see what future meds would be useful not actually dx crohns

At the time, 7 years ago, I was put on Pentasa and later added prednisone based of the IBD blood test. It just so happened that that came back the day I found out I was pregnant. After I delivered they did studies and scopes and found nothing. So I stopped all medications. I had on/off symptoms but nothing severe. This time around, it was clear that there was inflammation. Stool test and whatnot all came back negative. Since I responded to steroids both times he said he feels strongly it is IBD. So I guess I am not really sure what the plan it. I don't know if he will start different meds if the IBD panel comes back again consistent with Crohn's.

They don’t dx crohns based on ibd panel bloodwork. They have to have biopsies consistent with crohns or imaging consistent with crohns
Stool tests just mean inflammation but not what caused the inflammation (disease ,infection etc..)
Prometheasius panel or similar for crohns is not diagnostic of anything
One kiddo on here had a positive crohns blood panel
But did not have crohns at all
After years of chasing crohns dx and feeling horrid
Had immunodeficiency
Another kiddo similar was given UC dx
Nope turns out it was bechets

lots of inflammatory diseases respond to steriods
Auto inflammatory syndromes (many of those )
Eosinophilic diseases which can affect the gut
And many more
Which is why scopes abd imaging is do important
When time is critical our last Gi would admit my kiddo then scope the next day to “jump” the scope line
My kiddo was dx at age 7
Now almost 18 so I get his confusing the early days can be

they thought he has eosinophilic colitis at the tune before his scope
But it was crohns instead

can you get a second opinion ?

I understand that the IBD test does not diagnose anything. I also understand that a lot of inflammatory disease respond to steroids. I had imaging and am just waiting for scopes. I don't know about getting a second opinion as I live in a pretty rural area.
