Could this be Crohn's? Severe food sensitivities

My main symptoms are severe food sensitivities/intolerances. Literally, for the past 4 months I have been eating nothing but fish/salmon (fried in a little coconut oil) and more recently, some ground pork. ALL other foods make me gain weight (e.g., 1-3lb the next day), fatigue (depending on food, it can be debilitating), thirstiness, night sweats, sometimes severe cramps, nausea, edema, headaches, constipation alternating with watery/loose stool (no blood). Intially, I thought this was a case of severe "leaky gut", but it's not going away or getting any better even with an anti-inflammatory diet. I cannot tolerate any supplements (e.g., vitamins, probiotics).

I started eliminating foods in Spring 2009. First, gluten and corn (including corn derivates). This provided me with some immediate relief, but not for long. Over the following years, I eliminated every food except for what I am eating now. Even broth(!) bothers me and if a food isn't organic, the reaction is triple as bad. Even now, I am not totally sx free as I still experience fatigue after eating and nausea, C or D (never more than 2x/day) and fluctuating water weight.

I am seeing a specialist....In October, I started on a PPI and in December Sucralfate, which eliminated my slight acid reflux but that is it. What is interesting is that when the doctor presses on my stomach it hurts A LOT everywhere on my abdomen, even on my ribs. I thought if it was Crohn's the pain would be localized on the abdomen (e.g., bottom right quadrant), no?

Also, my blood tests are relatively normal. My ESR was only slightly elevated (23) and my IGA was only a little below range. I am a 26yo female. A Stool Analysis showed there was Occult blood in my stool but there were no significant inflammatory markers. I have no IgE allergies. I think next on the list is a Upper GI Barium X-ray said my specialist.

Could this be Crohn's?

Hi, Smax and welcome to the forum!

Overall, are you losing weight? I see that you gain weight after eating anything. That's interesting. Are you bloating at all? I'm guessing it's water weight?

Have you had a colonoscopy? It sounds like that would be a logical next step to take. Does your doctor think it might be Crohn's? Have they suggested any other possibilities?

No, definately no weight loss....just weight gain. Yes, I have bloating, although sometimes it's hard to distinguish from fat (LOL).

I have not had a colonoscopy. I was tentatively diagnosed with gastritis and gastroparesis but even with meds for those, I still have all the food sensitivities. The doctor is kind of puzzled by my sx.
 
Yes, and I had no IgE allergies. I did have a small welt to corn pollen and wheat, but the follow-up blood test to confirm if they were allergies was negative.
I didn't think I could be allergic to everything but fish and pork anyway, ;)

It is unlikely someone would be allergic to that many things, but it could be a few (or a lot) of very common foods or even spices or preservatives, making it very hard to determine. :)

It does sound like IBD is a possibility, but the weight gain is definitely unusual. Colonoscopy with biopsies would be the best way to determine. Hopefully they're going to get one lined up for you soon. :)

I'm going to tag David in this - he's pretty good with these tricky sorts of situations. :)
 
Hi David - No, I've never had a fecal calprotectin test done.
The stool test I ordered from my naturopath showed normal lactoferrin and lyzozyme levels and no WBCs.

In Sep 12, I had a B12 test done, which was slightly high. However, this was likely due to having Myers Cocktail IVs done biweekly over the summer.
In the past (say 5-6 years ago when I last had one done), I remember it was low.
 
I haven't had a breath test...not even sure if that is available in my city.
However, my stool test showed no overgrowth of bad bacteria. (I thought a stool test could evaluate for SIBO as well). Actually I was missing 3 strains of good bacteria ("no growth"). Seems like an easy fix...but I can't tolerate probiotics or even homemade sauerkraut.
 
Have you had your thyroid tested? Weight gain with low caloric intake could also be an indication of hypothyroidism.
