since I was diagnosed it was Ulcerative colitis. When I was about to get my jpouch the surgeon said I had crohns and I didn't agree and he wouldnt do my surgery. But after getting 4 different drs and another surgeon to talk him into doing it and 6 months later he agreed to do it. He basically the whole time saying this was a mistake and that I need to just have a perm ileo. So when he did the surgery and in recovery he came in to talk to me and he said that after looking inside he said I don't have much scaring and he now thinks its ulcerative colitis . How can you tell the difference? By looking how can you tell the difference between uc and crohns?
 
I believe you can tell the difference under a microscope (granulomas are Crohn's). Also, Crohn's is more patchy, whereas UC is more continuous.
 
I agree with chrisnsteph, visually with Crohn's you will see a skip pattern whereas with US the inflammation is continuous so maybe this is what he saw.
 
Oh. It was weird because when they took my colon they sent it to the lab. Seen no crohn's and then this surgeon said he looked at what the lab said and he thinks it crohn's. I always wonder why he would say this to me. Even his assistant did a biopsy of my rectum at the time and said it was uc and not crohn's and the surgeon wouldn't agree with him. I just wonder if the surgeon just didn't want to do it and finally agreed cause I wouldn't give up and tracked down every dr I worked with and the surgeon that did my ileo and made them call him..
 
I believe UC affects the colon whereas Crohn's can affect not only the colon but the small intestine, stomach, esophagus, mouth, and any part of the digestive tract.
 
I have and it ain't pretty. I asked for a pic after my last colonoscopy, the one they gave me not only had four different shots, it was in colour. Lets just say that when I got denied for disability, I sent them that pic and in less than a week I was accepted. I think it was the amount of red that got to them. lol

I have seen a you tube video of a pathologist explaining the difference between crohns and colitus is the layers of the large intestine affected. There is the mucosa and if that is all that is affected it's colitus, but if it goes deeper in the the other layers it's crohn's. here it is http://youtu.be/5WZNYP-sWnY
 
I think the crux of the matter is the presence or not of granulomas microscopically. I had a pan colitis, not much in the way of skip areas, ileum not affected, but because there were granulomas then it's a Crohn's colitis, not UC
