Possible Crohn's?

Okay, so... long story!

My mum has Crohn's, as does my first cousin (her niece).

Thinking back on it, I don't think I've had normal bowel movements since I was in my early teens (29 now). And I've certainly had sporadic abdominal pain throughout that times, which sometimes has me doubled over. I get lots of gas. Sometimes I go to the loo eight times a day, sometimes I go once or not at all. Usually it's loose, rarely it's hard, and even more rarely it's 'normal'. Even when it's loose, I have some difficulty passing. Occasionally pencil thin, usually broken up. Sometimes very pale. Sometimes nothing but a smearing of mucus on the paper.

Over the last year or so, I've started getting a pain in my left side, just under my rib. It nags and nags before I have a BM, and usually feels a bit better a short while after. When I lean forward or bend over forward, it feels like there's something there under my rib that's filling me a bit - like it will pop under my rib if I bend a certain way.

I also started getting, about 2 years ago, chronic urticaria. I take antihistamines daily to control it, and I spring up in hives if I forget to take one.

Additionally, I started getting really bad acid indigestion around a year ago.

I've explained these things to my doctors on several occasions, including times when I've needed to get hemorrhoid medication and movicol, but they don't seem to really take much notice. They dismiss it, so I figure it's probably nothing serious and drop it. The one time I was insistent, the doc had me lay down on the bed, prodded the bottom-right of my abdomen a little (throuh my clothes), then said "you probably have a little bit of IBS or something".

One thing - I am overweight. I'm in the 'obese' category, but not the 'morbidly obese'. I recall my mum and my cousin losing a LOT of weight before their diagnoses, so this makes me think it's not Crohn's - but then I also recall them both telling me they ignored symptoms for years too (when not doing so might have meant they'd both have avoided the major surgeries they'v had). I'm pretty sure the doctors take one look at me, think 'fat chick', and ignore the possibility of anything other than a crappy diet effecting my GIT. But the symptoms have been the same, even when I've been eating a healthy diet and getting plenty of exercise.

So I guess my question is - does this sound like an IBD? If so, how do I get my doctor to take next steps instead of palming me of with 'a little bit of IBS'?
 
I know this post is probably a pain, but any info/advice would be appreciated. I was laid up with abdo pain for the whole day after posting this.

Is there maybe a different section of the forum that my post would be better suited to?

With your family history, an IBD does sound like a possibility. Also with pain on the upper left, and pale stools, your gallbladder ought to be checked out. And IBS is a definite possibility, but shouldn't be diagnosed until a doctor has actually ruled out any other conditions that it could be.

I would keep on at the doctors till you get them to actually listen. If you have different doctors available at the same practice you can try each one in turn. I had to go through this with some neurological symptoms, it took me 18 months but eventually I found a doctor who would refer me to neurology (if only to get rid of me :))

Alternatively, you can wait till you are laid up with pain again, and try to get an emergency appointment so that the doctors can examine you when you are actually in pain. I say this because when I became ill I 'felt' the pain in the same place as you. When the doctors examined me, they found that it was my lower abdomen that was actually inflamed and reacting to their touch. Sometimes pain signals get messed up and we feel them inthe wrong place because our guts don't actually have pain receptors.

You could also try keeping a food and symptom diary, to see if there is any obvious correlation between what you eat and your symptoms, to rule out food intolerance (also some conditions like gallbladder disease are triggered by certain types of food). And keeping track of your symptoms will help when you go to see the doctor.
 
Tested for celiac? Pale stools are common in celiac.

Losing weight isn't necessary, especially in very localized Crohn's, where large portions of the intestine are not involved, as you would have lots of intestine to absorb nutrients.

With your family history and the skin issues, it would definitly point to a inflammatory condition such as celiac or IBD. I would push hard for tests to rule out IBD given the family history. Colonoscopy/Small Bowel Followthrough and test for celiac.

Thanks guys. You taking time out of your day to give me advice is really appreciated.

I plucked up the courage to go to the docs today. They scare the life out of me, so to avoid doing what I've done in the past and barely getting through one symptom before they tut and tell me it's IBS and shuffle me out the door, I took a list of symptoms with me.

Seems I didn't need to though. It's my first visit to a GP since moving to Ireland, and the difference was unbelievable. I was in there 20 minutes as opposed to the state-prescribed 7 in the UK that usually turns out to be 3.5!

Anyway, I went through everything. I have Fybogel with Mebeverine to take once per day until I see her again, I have to have fasting bloods taken on Tuesday for a general check, and a Celiac check as well. Plus she's ordered a colonoscopy and endoscopy for me, and will refer me to a GEMS practitioner if anything untoward shows up.

Carlsberg don't make primary care appointments, but if they did...
 
Wow! A doctor that listens to you :)

Make sure you eat plenty of gluten before your blood test, as the celiac check measures the immune reaction to gluten. No gluten means no reaction even if you do have celiac.

was interested when you said you've moved to ireland. Our doctors are good. The whole process is a bit slow but get yourself to casualty if the pain gets bad and they'll do all the tests you need while you're in. What part of the country ru in?
with regards to the left sided pain
does it feel like a large sausage shape. could be your bowel impacted and that can cause diarrrhea. i get pain there b4 a bm also and i have left sided Ulcerative colitis.

And no, no sausage shape on the left there. No anything in fact. Strangely, when she was pressing around on my tummy today, I felt pain at the bottom right... and the top left wasn't tender to the touch at all. Weird!

@ Rebecca - Thanks for the tip! I'd have forgotten since we're doing the whole low carb thing at the moment and I haven't touched anything gluten-y in a couple of weeks.
 
No one can tell from a forum if someone has crohn. There are a handful of diseases that have the exact same symptoms. Only way to be sure is colonoscopy with biopsy, it's the gold standard to see if someone has crohn.

If they do colonoscopy ask for biopsy, you do not want to do it twice because the doctor forgot. Hospital here will not even treat people before biopsy is done.

Bottom right is terminal ileum spot (where colon and small intestine come together), most people with crohn have terminal ileum lesions (and often pain on right side). Not good enough for diagnosis, explaining so you know where the pain might be.

Just a thought if there's inflammation in the lower part of your sigmoid colon or rectum it would hurt as the poo passes through.
