I'm just wondering why more people haven't had the Prometheus IBD Serology 7 blood test. I didn't even know this existed until a few months ago. I met a new GI doctor who immediately tested me in conjunction with giving me a new colonoscopy and upper endoscopy.
The prometheus lab came back saying that it was "predicted" that I have crohn's. I'm just curious because it seems from a lot of the threads that many patients are still in limbo-they don't know if it's UC, Crohn's or something else. According to the website below it helps determine which one is more likely with a pretty high percentage.

PROMETHEUS® IBD Serology 7 helps identify IBD and differentiates between ulcerative colitis and Crohn’s disease with an overall diagnostic accuracy of 89%. This test includes the proprietary and patented markers anti-CBir1, anti-OmpC and DNAse-sensitive pANCA process that helps identify patients with IBD and utilizes Smart Diagnostic Algorithm technology to improve predictive accuracy. While most other labs only offer assay values, PROMETHEUS IBD Serology 7 provides a diagnostic prediction on every test and prognostic information that may guide treatment decisions. For references please see Product Information Brochure.

I know that it's fairly new but my guess is that it's an insurance thing. The tests at that lab are expensive. That's the same lab my blood went to orginally when they were getting me ready for azathioprine.

I have had this test done twice. I switched docs/insurance at one point. Both times my tests results came back negative for both crohns and colitis. However, based on what the docs had seen (irritation, swelling throughout bowels in particular the terminal ileum) they wanted to treat me as a crohns patient.

That being said, I have never taken anything until present day to maintain remission. I have flare-ups very rarely (1-4 years apart). But the last two I had 2003 and November 2007 were severe enough that I rec'd meds prednisone/remicade then prednisone, respectively) to come out of flare-up. Then took myself off of them once I was symptom/pain free....Well, I am still on 10 mg of pred, since the flare-up in Nov., as I am still weaning off......

I had the test. Mine said "not consistent with IBD." My GI said we can see from your colonoscopy that you for sure have IBD so the test was not helpful.

Yes, I've had it twice (consistent with Crohn's), with about a year in between, and my doctor seems to think I'm one of the rare false positives (which I doubt). He claims the test means nothing as far as a diagnosis, and is only used to support other tests.

I had a blood test looking for "markers" of Crohn's. This was after my diagnosis by biopsy. The test came back negative. I don't know if it's the same thing or not.

I had the prometheus test 1 for IBS and 1 for IBD his dingbat nurse did the IBS by mistake the IBS came back normal and the IBD came back strongly consistent with Crohns he showed me the test and I read it myself.

Never had it. Though they've either been in my bowels or in my stomach cavity so many times that it's pretty much a 100% foregone conclusion as to whether I have Crohn's or not. Sometimes you just don't need redundant confirmation.

I had these tests done. Requested a copy from my doc. I'm not yet diagnosed with anything.
Here are my results:

Assay Assay Value Reference Value
ASCA IgA <12.0 EU/mL <20.0 EU/mL
ELISA

ASCA IgG 20.2 EU/mL <40.0 EU/mL
ELISA

Anti-OmpC 16.1 EU/mL <16.5 EU/mL
IgA ELISA

Anti-CBir1 28.3 EU/mL <21.0 EU/mL
ELISA

IBD Specific pANCA

AutoAntibody <12.1 EU/mL <12.1 EU/mL
ELISA

IFA Perinuclear Not Detected Not Detected
Pattern

DNAse Not Detected Not Detected
Sensitivity

If I am reading correctly, it's only that one value that's out of range, but results say "Pattern Consistent with IBD: Crohn's Disease." I guess that's why my doc said it was "weakly positive."
I tried to format this to look like a chart, but it didn't work. The first value on a line is the assay value and the second one is the reference.

my test came back consistent for Crohn's but I'm not sure how useful that was... They had already diagnosed me with Crohn's since I have narrowing in my small intestine.

I was first give a colonoscopy during a visit to the hostpital for extreme rectal bleeding and blacking out. My GI doctor that they oppointed to me said that I had ulcerative colitis of the whole colon. After seveal days in the hostpital and several months in really bad shape it was about 4 weeks after I got out of the when I had this test and it can back as showing I had ulcerative colitis. So it basically confirmed what my GI doc all ready knew.

wowie im special, my vote made the results 50/50!!
anyways yes i have had the test done, many many years ago....i think it was 05? ok well not that many years ago, but its a while i guess...

um to be honest i dont know what my results were and im kind of confused on the whole thing, i thought the reason i had it done was to check my system for remicade antibodies, cuz my remi wasnt working anymore. i thought thats what the prometheus test DID?

hmmm....i guess i missed some things there lol

NOTE::

i think the reason a lot of people havent had it as a diagnostic tool is cause it is costly. if im not mistaken, there is one lab out in washington state that processes these tests, somehow they own the technology and are the only ones allowed to process these labs.
SO if you live on the east coast like me, or hell even out of the US, thats a looooong way to ship a blood test, which equals money.
i am lucky and am covered under good insuance thru my dad, but i know that coverage is a huge problem for som many of you, and a test like this is a huge luxury.

Yeah, I am confused as well. I had a test prior to starting Remicade and it was sent to Prometheus labs. I know I had to pay like $200 out of pocket for it. As far as I know, it was just to check to make sure I could take Remicade. Not sure if it is the same test or not.

Looking at their website, they offer a bunch of different tests. The particular one that this thread was about is their IBD serology 7. One thing it is supposed to do is help docs distinguish b/t UC and UC-like Crohn's. I guess it would also be redundant in a lot of cases - if you're a confirmed Crohn's sufferer and there's no question about it then what really would be the point?

Yeah it was expensive. I shelled out $1200 for the IBD serology plus the Celiac test. That was not a good day.

I'm pretty sure this is the last blood test I had done when I first ended up in the hospital with my abscess (before I knew I had Crohn's). I am not even sure I ever got the results of that test. Ha!

I assume it was the Prometheus test. i think they had to send it to California and it took a week to get results.

hahah shady we are like always in the same boat. long lost crohnie twins lol

Had one earlier this year when I went to the specialist at UCLA. He was not sure if I had Crohns or UC based on my colonoscopy history.

He said it came back as intermediate Crohn's. Mine is all in my colon and has never shown up in my small intestine.

Thanks for all of your responses. My doc ran this test because he was trying to determine if my diagnosis should be changed from UC to Crohn's. Which after this and 3 other glaring facts helped us change the diagnosis to Crohn's.

I'm feeling a little anxious because I haven't taken my Humira in about a month (random, I know). I had to stop to have ankle fusion surgery :/ I better call and get some more shipped to me...

LOL Kello. I've hardly been through a fraction of what you have. But it's nice to think I have a Crohnie twin!;)

I had some kind of Prometheus test done back around 2001/2002 if I remember correctly.....unfortunately, my insurance DID NOT cover the testing, and I didn't find out until I got the bill!!!!.....apparently the Dr office did not check before ordering the test, and I didn't think of it as all my other testing is covered...

Don't remember what the results were though.....probably Crohns, as before then it was always a toos up as to if I had Crohns, colitis or both.....

Just to let everyone know...if your insurance denies coverage for Prometheus testing you can appeal it. Prometheus offers sample letters of appeal on their website you can send to your insurance company.

Ameslouise told me about this, so you can thank her for the info. I'm just passing it along :)

ASCA IgG ELISA < 12.0
Anti-OmpC IgA ELISA <3.1
Anti-CBir1 ELISA 8.9
