Propel a Cure met in a virtual meeting with Dr. Pradipta Ghosh, a clinician scientist and professor in the Department of Medicine and Cellular and Molecular Medicine at the University of California, San Diego. Also interviewed was Courtney Tindle, the Program Manager and Director at the Humanoid Center of Research Excellence at University of California, San Diego and the project lead for Crohn’s disease.

@kiny Do you think the ones that are able to manage their Crohn's purely with EEN or diet belong to the subgroup that is immune-deficient infectious-CD (IDICD)? The accidental discovery of the two subgroups using organoids is fascinating.

The people who seem to respond best to EN are those with classic crohn's disease.
-age of onset around puberty when peyer's patch activity is highest
-ileal enteritis with minor or no colon involvement
-often presents with granuloma at some point in the disease history
-often primary immunodefficiency in NOD2 and ATG16L1 (are sometimes diagnosed with primary immunodefficiency diseases like chronic granulomatous disease)
-often positive for ASCA test
-sometimes positive for anti-OmpC
-sometimes penetrating disease

This group also responds very well to anti-TNF, which can be combined with EN.

It is also in this group that AIEC are found that invade M cells like salmonella.

The moment the colon is involved, everything seems to change. I don't think people with colonic involvement and no ileal inflammaton, so-called crohn's colitis, have the same disease.
