Tendonitis and whole body aches with Cimzia

I’m a 68 year old male. Diagnosed with ileitis in 1979. Survived for many years on Sulfasalazine. Had diarrhea and other IBS symptoms for many years. In 2004 had bowel obstruction resulting in surgery. After that, began having several other obstructions and was on and off Prednisone. Went on Humira in 2013. Felt great for a year or two. Then went off it and back on it several times for hip replacement surgery. GI doctor said I’d be fine .... he was wrong. I developed an immunity to it. In late 2016 switched to Cimzia. My GI situation is great .... diarrhea gone. However, in last 6 months, after being on Cimzia for 14 months, began having all sorts of tendonitis problems. Affected both shoulders last Summer. Had never had a shoulder issue in my life. Now have it in upper IT Band where tendons meet the pelvic bone at hip. Also for last 3 months have a totally achy lower body from the hips down. Aches all day long. I’m currently taking Hydrocodone which takes the edge off but fairly ineffective. Have 2 Pain Management doctors. I’ll be seeing a hip specialist soon. Have been to a Sports doctor. He’s stumped. I am totally disgusted. There is no joy in my life .... just pain, pain, pain. Will need a psychiatrist soon if no relief. Hate to stop the drug .... maybe this is all unrelated and I am just unlucky. Can anyone help me? Thank you.

Have you seen a rheumatologist?

Spondyloarthritis is very common woth crohns and can cause issues with the hips and especially the tendons

My kiddo has the juvenile version
 
Hi Maya 142, my little penguin and Ron. Thank you for the responses. I neglected to mention that I have had a total hip replacement in both hips which makes this all more maddening as I thought my joint problems associated with the hips were totally over. Walked 4 miles last year at Epcot..... now can’t go 4 blocks. Friend says problem emanating from SI joints so will mention that to pain doctor Friday and he may try injections. Never had hip tendonitis in my life. Now left side is bad and have just in the last week started having pain on right side which was 100% okay since replacement surgery in 2015. Just frustrated. Sorry for the negativity. I went to a Rheumatologist last June for the shoulder tendonitis and severe neck problem I have had for years. Put me on 5 mg Pred per day and 500mg Sulfasalazine. I’m okay sleeping and get achy after an hour or two of being up. Then ache all day. Have had Crohn’s 40 years ...... never had these symptoms. Had blood test for Lyme’s Disease yesterday ...... long shot by doctor I think but who knows. How are the Stelara and Entyvio treatments working? I will probably try one of them if I develop immunity to Cimzia. Thank you for the arthritis info. I will research that. Hope you all are doing well.
 
I had the same problems on Cimzia. I went humira= antibodies to cimzia for only a few months (because of the body pain) then to remicade. I still have a lot of the joint pain and get shots in my right hip a couple of times a year. At 51 maybe it is age but I never needed them before the cimzia and it hasn't cleared since ending cimzia over 18 months ago. See a rheumi. Also I had carpal tunnel surgery after stopping cimzia. Maybe connected- maybe chance. Good luck.

Entivyio doesn’t work if you have arthritis at all
Stelara is ok as long as you don’t have the spine involved
But takes 6-8 months to work

Hang in there!! A LOT of what you said sounds like it could be spondyloarthritis (SpA). It's a type of arthritis that effects where ligaments and tendons insert into the bone. That is called enthesitis. Heel enthesitis is very common, as is enthesitis around the knees, pelvis and in the feet/ankles.

SI joint arthritis is a key part of spondyloarthritis. In both axial spondyloarthritis and Ankylosing Spondylitis, the SI joints are involved. I would definitely see a new rheumatologist as soon as possible. And I would not do steroid injections into your SI joints, because that will mask any possible inflammation.

You do have a number of symptoms that are red flags for AS/SpA - significant hip involvement (my husband has AS and has had 5 replacements starting at age 30), SI joint pain, psoas tendonitis (enthesitis is also common with the psoas tendon - the point where it joins the bone becomes inflamed) etc.

Stelara works for psoriatic arthritis, but not AS. So it is important to figure out what you have before your treatment plan is changed. Another good reason to see a new rheumatologist ;)
