I like this one written for older elementary and teenage kids:

Coping With Crohn's Disease and Ulcerative Colitis by
Christina Potter

It seems to be out of print but can be purchased used quite easily online.

We have read the Toilet Paper Flower with our 10 year old. It does explain to her friends what is going on with her. We'll have to check out the other books listed here. Thanks for the info.
