I wrote an e-mail to my infection and clinical microbiology doctor, I'm in a bad flare right now and asked him about clarithromycin and rifabutin combination. Sent these articles to him: http://www.ncbi.nlm.nih.gov/pubmed/9096189 , http://www.ncbi.nlm.nih.gov/pubmed/11930899

Contraindicated

clarithromycin + rifabutin

clarithromycin increases levels of rifabutin by decreasing metabolism. Never use combination.

Significant - Monitor Closely

rifabutin + clarithromycin

rifabutin will decrease the level or effect of clarithromycin by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Potential for interaction, monitor.
I know they use subtherapeutic doses in RHB-104; but what do you think? I see RHB-104 as a good option for the future and want to know.

2 things come to my mind...

Unless I am misreading the response it appears that the only effect is decreased metabolism?


And my last thought is in reference to the understanding that they are 'orphaned drugs' that were FDA/NICE approved and were used to treat leprosy.


Do you have any thoughts yourself on your findings?

Does anyone else have anything they can add?
 
I'm sorry for the late response, I've had an internet connection problem. I don't have any thoughts about this nor any good info. This topic seems confusing. Maybe someone knows and write a reply.
 
I was on this drug for literally 1 week at the lowest dose. I stopped because the drive was too far and I also thought it was effecting my appetite.

My stool tests came back from my last visit and I've tested positive for cdiff. Im not having any diarrhea or weight loss. Not really showing any symptoms of it. So I guess it could be a false reading. But the rest of my stool test and bloodwork seemed to be consistent.
