Bad flare for the last month - need med ideas
Hey all. I'm new to the forum but looking for any advice. I am in the middle of a bad flare up. I was tapering down prednisone when it happened and got off rowasa enemas. I'm am now back up to 40 mg of prednisonr, metcapurtorine, asacol, and hydrocortisone enemas but nothing seems to work. I have left sided uc

I cut out caffeine and carbonated beverages. Seems like anything I eat causes a reaction. Have loose stools for past month and bleeding.

I know everybody is different, But about three months ago I started the Paleo diet and it is made a huge difference for me. Cutting out all wheats/grains and dairy has been a savior. It might be worth a try. If you do try it, let me know how it goes.

You might need to try Humira or Remicade. Have you contacted your GI?

Yes, going for a biologic is definitely the most prominent choice.

Just to add to this, the other things you might consider are
a. what dosage of 6mp (mercaptopurine) are you on in mg/kg or mg/lbs? If the dosage is too low, it might just be that 6mp is ineffective. Also, how long have you taken it and are you consist in taking it (compliance as in daily, splitting the dosage to 2 times per day)?
b. how much asacol are you taking per day? Are you taking them orally or per enema?

As people have said above, other things to try is to adjust your diet. If you want a more radical approach on diet, you could go for EEN (exclusive enteral nutrition - basically you just go for drinkable supplements/formula for a few weeks) or do that partially. There are various other diet suggestions that have had success for acute flare-ups.

Lastly, there are of course a few other things as well, as medical marijuana that can help, or switching from 6mp to a different immunosuppressive (methotrexate - MTX) or even combination therapy, that is MTX and a biologic drug.

Having said all that, it is always a good idea to read up on the various threads that others have started on this forum. Reading many threads with the various approaches people have taken really gives you a great overview of what is out there and what has worked for others.

I'm on asacol 800mg 3x per day. I have only been on mp for about a week now. And my dosage is 75mg 1 x per day. Is that low? This is the first immunosurpressent I have been on.

In that case, give the 6mp some more time. It can take several weeks sometimes before you see the full effects of it.
