First day on Prednisone...Any advice for me?

Flare began in earnest in late July. Max dosage of Colazal wasn't working. So, I'm now starting my steroid treatment.

Prescribed 40mg/day. Took first dosage tonight at dinner. Seeing doc in two weeks to reevaluate dosage.

I read and hear the side effects can be brutal. I'm a bit scared to be honest. Just hope it's worth it in the end.

Any words of encouragement are appreciated!
 
I got a moonface and acne, and gained weight (maybe just because i was eating again, lol). i also had some really vivid and SCARY dreams. i don't dream that often, but every time i have been on pred, i get bad ones!
 
Isn't it generally easier on yourself to take it in the morning? That's what my husband has always done when he's had to take it.
 
I hear that it interferes with people sleeping. You might want to slowly switch to a breakfast dose to help with that. I've been on it several times and had no bad side effects from it. I did gain 40 pounds on it the last time, but I was trying to gain 30. I actually feel great on it - I finally have energy. No moon face so far, and no extra hair. I just started 40 mg 3 weeks ago. I'm supposed to taper weekly, but stayed at 40 for 3 weeks just because I'm finally feeling a little better. It's different for everybody, you might not have any sides at all.

Thanks for the quick advice, all. I'm taking 2 pills at breakfast, 2 at dinner.

Maybe the acne will make me feel like a teenager again! And, I've always been thin (5'9, 155lbs) so some weight gain might not be so bad.

Did you see immediate improvement in symptoms? I've been at K-4 everyday since July (see my post in "Your Story" under "My Life with Bad Gas" for my poop scale).
 
I am not on prednisone, but am taking Entocort. They told me to take all the pills in the morning to make sure I could sleep better. I felt better right away. Been hungrier, but not putting on the weight I would like yet.

Hope the prednisone works for you. I have had other friends who were on it and didn't mind it so much cuz they felt so much better.
 
I've been on prednisone for the last month or so. I started at 40mg as well.

1. Make sure you are eating lots of potassium. Potassium deficiency is very common with prednisone and not something you get warned about very much. Potassium deficiency can be very bad for you as it spikes your blood pressure and can do long term damage. Monitor your blood pressure with an automatic machine at home or at the store or even better manually if you know someone who can do it.

2. Prednisone tastes BAD, and it dissolves on contact with your saliva. Take it with pudding or yogurt (or mayonnaise if you hate sweet things). The trick is that you need a thick liquid that you can swallow without any sort of chewing. It both helps grab the pill so that it goes down ASAP, and it will coat the pill so that when it dissolves it has a barrier between your mouth and the pill.

3. Your immune system is weak. Especially to cuts and scraps. If a wound may have gotten infected before the prednisone, it WILL get infected now. It's also going to take you longer to get over illnesses. (For example I'm worried this bloody flu I got is going to need antibiotics to avoid pneumonia) Take care of yourself and avoid catching illnesses and infections.

4. Take a multivitamin. I could have included this in a couple of the other points but I want to give it special emphasis. Your absorption of various things is being countered by the prednisone and you'll need things like vitamin C to strengthen your immune system's other abilities to compensate for the lack of inflammation. You will not get a significant amount of potassium from an over-the-counter vitamin. It can't be put in pill form without making the pill taste so bad no one would buy it. You still need to eat potassium rich foods.

5. If you stay on this medication long-term you will get osteoporosis and much earlier than normal. Prednisone will cause your bones to hollow out like a bird's but, very sadly, it won't give you wings and feathers.

6. Ask your doctor about sleeping pill options if you're having trouble sleeping. Twice daily dosing is good for you in reducing side effects and helping to keep your adrenal gland healthy but it can cause some really nasty insomnia. Your doctor will not be surprised and sleeping pills of some sort (maybe even just a melatonin supplement) can help overcome the problem.

7. DO NOT STOP TAKING THIS DRUG AHEAD OF SCHEDULE AND DO NOT MISS DOSES. Understand? Prednisone is produced naturally by your adrenal gland in much smaller amounts (0.5mg equivalent I think). Your adrenal gland works on a feedback loop with short-circuit protection. If there is more prednisone in your system than normal it will shut down. Adrenal gland no work. But, it requires some prednisone to get started again. So, if you stop taking this medication without following the proper dosing your adrenal gland will not start back up and your heart will run out of adrenaline and slow to a stop. The longer you're on it, higher the dose, and the older you are, the more difficulty your adrenal gland will have getting started again.

8. Prednisone is awesomely effective for most people. Just like any medication, if it has a strong positive effect of your body it's probably going to cause some collateral damage. For short-term use prednisone is a great option. Just try to find something else to take long-term if this doesn't put you into remission for a while.
 
I want to echo Colt's #7 point, it happened to me because I knew nothing about the drug and my doctor didn't tell me. I went from 40mg to 0 overnight and I spent 4 miserable days hoping I would just die. It was one of the most unpleasureable things I've ever went through.
 
wow i never knew how bad just stopping it could be.....i was told not to do it no matter what but now im definately making sure i dont miss a dose.

i think overall taking pred is worth it, im on it now and my cheeks are like a squirells and i hav a moon face but my crohns is much better than it was and in the end that matters more than appearance. the worst side affect i'd say is the sleeping.....i highly recomend getting some sleeping pills.

Thanks! I'm not on steroids yet, but the doctor is looking at Entocort or Prednisone...the last time I had prednisone I got the shakes too, so maybe Entocort this time? I hear it has less side-effects...
 
Thanks all! Great advice - some I knew already, but some I didn't.

I'm on Day 3. Feel super energized. Work colleagues think I've been drinking double venti latte's at Starbucks. Getting 3-4 hours night sleep yet still feel energized.

Hoping to get some napping in this weekend.

That said, still no improvement in the poop department yet :(
 
Entocort is localized to your intestines so it should have very few side-effects compared to Prednisone. Unfortunately, it's localized to a specific segment of your intestines (terminal ileum and ascending colon) so it won't help outside of that area. It's good for some people, not good for others. It all depends on your disease specifics.
 
Iv been diagnosed with chrons disease 3 week ago. I know nothing about it what so ever. but i know the pills i have to take are called prednisone steroids. iv put on 3 stone in the week iv been on them im constantly hungry. (not knowing this is a side affect till reading this thread) Also never knew that ya cant just stop taking them either. whew! but other than being constantly hungry no side effects. I feel invinceable on them full of energy and i can stay awake now and not sleep as much as i used to.
 
While you are on Pred eat a high protein, high Calcium diet. Take Calcium supplements (preferabley Calcium Citrate) and a multivit; make sure to get enough vit D from these (400 IU is 100% RDI)
 
Sleeping pills. Try melatonin, then benadryl, then go to prescription stuff if needed.
