Help! Sore butt feeling miserable is this crohns?

Hi so sorry but I am so sore and miserable can anyo e advise please? Basically thought my post childbirth piles had taken a horrible turn for the worst loads of bleeding plus agony durin bm and hours after. Anyway saw colorectal surgeon early July diagnosed fissure and prescribed anoheal. Cream nt working and I can't cope with this pain. Could it be crohns? I've had diarrhea daily for 6 yrs colonoscopy 6 yrs ago diagnosed with ibs which I never believed . Never get constipated so assume fissure/s caused by years of frequent bms. Anyway the fissure is about quarter past maybe I have several? Next appt with crs in sept but the pain is horrible and the cream isn't working nor did rectogesic. Could this be crohns? Any help appreciated many thanks xxx
 
Fissures are a frequent issue with perianal crohns. It's like pooping fish hooks and razor blades. The pain may drive you back to the CRS before Sept. And be careful if you try not eating, so then you don't poop to try and ease the pain. You will still poop - just less frequently and you will end up depriving your body of the nutrients it needs to fight.

There are a number of things you can try to ease or minimize the pain.
Lignocaine or lidocaine applied topically can deaden the area.
Calmoseptine can soothe the area.
Use baby wipes to wipe.
Have a sitz bath after every BM if you can.
Lac-dol or a stool softener to ease the exit.
I could only poop in my shower with steaming hot water being shot on my anus with a baby wipe handy after each BM. If you reach this point - you're in strife.
You might want to speak to your CRS about a Botox injection in your sphincter to ease the spasming that generates some of the pain.

Have you had a formal crohns diagnosis?

I hope some of these suggestions might help you.

Yes, it could be Crohn's Disease or it could be Ulcerative Colitis. Daily diarrhea for six years along with your current symptoms make me think you should request a colonoscopy be performed to see what's going on in there.

Thank you so much I can't believe how quickly you replied. It is great to be able to talk to someone! I am in the UK so not sure if I can bring my appt with crs forward on nhs. He did say use the anoheal for 8 weeks and it's only been four but these fissures hurt like nothing else! I had a colonoscopy about 6 years ago when symptoms first started I was seen by an endo at that time because I was. Vomiting daily as well and also had a gastroscope. The upshot of it all was is that the reflux was caused by muscle in gut not working properly to keep acid in stomach and the diarrhea was Ibs he said there was some inflammation whatever that means? Six years on do you think they will do another. Olinoscopy on nhs? When the crs saw me four weeks ago He only looked at the fissure on the outside. I was glad at the time because even that hurt. When I use the. Cream it feels like I have more than one up there? Could they diagnose crohns by just looking inside without full colonoscopy? I was so desperate last night I took an anti diarrhea tablet :(( cant believe I am going into such detail but you just cant with family and friends it's so horrible and embarrassing. Thank you again and I will try the advice given but what is a sits bath and where do you get them from in the uk? Thanks again xxxxxx
 
Ps sorry about spellings in previous post - trying to do this from my phone with fat fingers !! I saw a gastroenterologist six years ago not an endo, seeing endo for high blood pressure could years of bad guts trigger high bp I wonder? Who knows I think I am falling apart bit by bit !
 
Sitz bath is just a very hot/warm bath. Some folks throw in Epsom salt.
You just sit in it and feel the relief.
You can get a little square bucket to just stick your bum in if you don't have a bathtub.

Your skin is probably very very swollen - that's why there might feel like there is more than one.

You probably need to see a GI to find out about getting an accurate diagnosis.
Topical cream is not going to remedy your situation. It will only limit your pain - rather than treat the cause.
Please be very careful and if it gets worse - go to the hospital.
An untreated fissure can do a lot of damage if left untreated.
I ended up with a Stoma.
 
Oh samboi I am sorry to hear about that I can't imagine what that must have been like. Will try the site bath think I will get a lock on bathroom door first lol! Yes it is swollen down there it hurts using the cream there defo feels like one at three o'clock that's the really painful one and then I can feel one on the outside at 12 o'clock and three o'clock. Never thout I would be thinking of my ass as a clock ! Know what you mean about razors and fish hooks you got that dead right and horrible throbbing after. I feel so sorry for myself and anyone else with these blighters! Thank you I feel better moaning about it I wish I had gone to my doctor sooner I've had this for probably a year I thought it was piles. Xxxxxx
 
My appointment has been moved to the end of October. I am do fed up with the pain:(( I definitely have two fissures the one st the quarter past position hurts the most is it unusual to have more than one? I don't know what to do with myself sometimes. Sorry need to moan I can't believe people suffer like this and there doesn't seem they can do much about the pain:((
 
Hi my appointment was 23rd September the clinic is cancelled so I have to wait for the next one at end oct. my first hospital appointment with crs was early July I have been using anoheal do this is a follow up from the July appt. Prior to that I saw my gp in may who prescribed gtn I don't think these creams are going to heal me.
 
Update, saw CRS today, fussure hasn't healed I am booked in for general anaesthetic so they can examine me properly and give Botox injection. The fissure is at quarter past position. Any ideas why I need a general anaesthetic for Botox? I didn't think to ask :((
 
Thank you, I am feeling worried about it but relieved at the same time that they are doing something. I will write questions down next time! Do you have Amy suggestions about what I might ask ? I am so grateful for your help:)
