How do I get copies of my records?

I know this differs with each hospital/insurance company/state....But I am just looking for a general idea. How do I go about getting a copy of my medical records? I would like to have them, because I've had some crappy doctors and want to make sure they didn't miss anything. Any tips?:eek:
 
If you're in the US, you have a legal right to copies of all your medical records. You need to put a request in writing with your signature. Each doctor's office or hospital will have a form that you will need to fill out and sign. I'm not sure if it's still this way, but several years ago, they could also charge you a nominal fee to make the copies; that many have changed. Also, expect it to take some time to actually get the copies. Good luck!
 
You have an absolute legal right. For your entire medical record you probably need to put in a request on a form they provide and may need to pay for copies. I believe NY state law doesn't allow a charge of more than 75 cents a page. They can take a few days to give you your record but have a set amount of time by law.

If you want individual records like your last colonoscopy they will probably just print it for you and not bother asking for any money or forms. That's in my experience atleast.
