Hi everyone! I am new to this forum and I have a question. I have had Crohn's since 2007 and once I started on Remicade in late 2007 it was well managed. However after having my son the Remicade started to lose its efficacy. I ended up with a microperforation in May and was able to avoid surgery through use of antibiotics and various other meds. I went to Mayo and recently started on Humira. Here is my question. I have started having the pain in my right lower quadrant again. It is nowhere near the pain it was originally but it is there. So how long should it take for this perf to heal? How long for the Humira to take full effect? Thank you!
 
I'm curious, how did they diagnose a microperforation? What symptoms did you have?

When I started humira (march I think) it took about 6 weeks for effect- less bleeding , less pain and vomiting etc but didn't achieve full effect for me so this week I have gone onto weekly humira (also on aza and pred).

I had an MRI followed up by a CT scan. My labs were awful and my pain was pretty bad. I had a follow up MRI a couple months ago and they said it was getting better but not completely healed yet. I also had another colonoscopy and they said there was still so much inflammation in my ileum and cecum that the couldn't pass the s ops through it. Just hoping the Humira works!
 
Hi Amber, when you get a micro perforation, there is a contained infection (abscess) on the outside of the bowel, unfortunately this can be quite "sticky" and join a few loops of bowel together in an inflammatory mass. This can also lead to internal fistula and also stricturing.

Usually internal fistula would either be asymptomatic, or have more diarrhoea. Stricturing however would cause pain, usually obstructive in nature, usually worst after meals. If they can't get into the terminal ileum, the it might well be strictured. If the Humira doesn't work, I would suggest considering a right hemicolectomy and continue the Humira to prevent recurrence of Crohns at the surgical join. However, if the Humira starts working, it may well be that you had inflammatory stricturing, and it should settle continuing the biologic therapy.

Thank you! I'm really hoping the Humira will take care of it. I am trying to avoid surgery if possible!
