    Mirikizumab

This is great news - I wonder, if someone would fail another IL23 inhibitor, like risankizumab, would they still have a reasonably good chance of reacting positively to this, because of the slightly altered mechanism - IL23p19 instead of IL23A ? Was wondering the same about ustekinumab vs...

EEN makes things worse - any advice?

each of us has a limited time until the current meds will work, and we dont know in advance if its 2 years or 50 years - none us can know this in advance - I have talked with more than a plenty cases in different diet groups online who have exhausted all their treatment options, and then its...

EEN makes things worse - any advice?

Thanks for your response and experience! Unfortunately (from this POV) I am living in EU so Peptamen is not accessible, I could get my hands on Kate Farms but it gave really bad pain and diarrhea. Nestlé Compleat worked beautifully for me, but unfortunately, despite going to GREAT lengths, I am...

EEN makes things worse - any advice?

So I have ran a new experiment with exclusive EN given I have ileal Crohn's - Modulen, trying to see if I react better to it then last time. From all the formulas I have tried (and there were elementals too) my gut seemed to have tolerated Modulen the most so I thought I will go with that...

    What Next?

I also didnt have granulomas on my biopsies, and I have Crohns. Significantly elevated calpro (especially if its persistent), inflammation in both the colon as well as the terminal ileum as indicated by multiple examinations that also lasts over time, malabsorption, as well as enlarged lymph...

First JAK Inhibitor Approved by FDA for Treatment of Crohn's

Damn, sorry to hear that! I am a bit confused tho - I thought that you cannot really become "resistant" to JAK inhibitors like anti-TNF-s due to the different mechanism of action - I guess naive on my part!

Inulin fiber exacerbates inflammatory bowel disease, study finds

Random thought: Pediasure and Ensure (very common EEN-s) actually have FOS in them.

Inulin fiber exacerbates inflammatory bowel disease, study finds

I am typically sceptical of these fiber = inflammation studies, as it does not overlap with my personal experience, but there might be something in above ones. I checked and I am actually not able to tolerate most (perhaps all) foods having inulin and / or FOS and / or b-fructans. The...

blood work result please explain my concern

Regarding those calpro numbers: I have what they called a “mild” disease - at diagnosis I had 2 small superficial ulcers in terminal ileum, nothing visible in colon, not much apart from a small and mild thickening of TI on MRE, and 600+ calpro. So always understand these results in her context...

blood work result please explain my concern

Remember most of us are not doctors here, so definetly try to find a chance to discuss it with him - and potentially ask for a second opinion too. Having said that: ERP and CRP being normal may or may not indicate anything about her inflammation status. I can tell you that I never ever had my...

    Therapeutic drug monitoring in anti-TNFs?

Great, thanks for the info! I live in Europe so hopefully state healthcare should account for it, lets see..

    Therapeutic drug monitoring in anti-TNFs?

I have celiac disease since I was a little kid. This is one of, if not the main gene mutations causing that, and one of my doctors was dumb enough to try to convince me I dont actually have celiac disease anymore as I didn't have symptoms (being on a strict gluten free diet lol). As a proof he...

    Therapeutic drug monitoring in anti-TNFs?

So I am a HLA-DQA1*05 carrier (a gene mutation present in around 30% of Europeans, and quite substantial in the Western world in general) and have read in many recent studies that unless something called "therapeutic drug monitoring" and "proactive drug optimisation" is used, the risk of...

The decision : On Biologics or Not

I agree with Entyvio not being too effective esp for non-colonic disease (perhaps above numbers would have looked a bit better if they would have looked at Crohn's restricted to the colon?) unless patient can somehow afford to wait quite long. But I wouldn't say the same about JAK inhibitors...

How common is to adjust Humira dosage after first levels checked

And for those of you who responded, did the anti-TNF start to work properly and for multiple years without loss of response after early dose escalation?

The decision : On Biologics or Not

I was diagnosed with very similar findings as yourself 1.5 years ago. I am trying to manage it with strict diet (and by strict I mean.. currently I eat the same homemade enteral nutrition every single day, and nothing else), not because I do not believe in biologics, but because we all have a...

Role of diet in prevention versus treatment of Crohn’s disease and ulcerative colitis

This mycotoxin stuff is interesting. Especially that I am currently eating 150g+ peanut butter a day in my homemade EN, and peanuts seem like the top offender in this regard. I might give it a try to replace it with something else and see if I do any better in a few weeks.
