Are these flare ups or adhesions

It has been a long time since I visited these boards and I hope you don't mind a bit of TMI! I was first diagnosed when I was 14 (I'm 38 now) and symptoms were predominately vomiting and pain. I had a number of operations between the age of 14 and 24 for a fistula, 2 resections and a couple for division of adhesions. My last resection was in 2000.

I feel like I have lived so long with diarrhoea and pain that I have lost sight of what could be a flare and what could just be adhesions. For a long time I have had cyclic symptoms in that I would have 2-3 weeks of pain and diarrhoea and the day I get my period the pain would be gone. The week of my period and the week after would be my good weeks and life is relatively normal. The last few months the pain has intensified. I wake several times a night in pain, sometimes so bad I feel like vomiting and I'd sweat. It starts to ease when I pass a BM. Sex during the bad weeks is excruciating and would result in me being doubled over and needing pain meds. I have put it off so long because I didn't want more surgery but I can't live like this anymore.

Now for my questions:
Does anyone else have symptoms follow their monthly cycle?
Does it sound like a flare up or is it most likely pain caused by adhesions?

I have an appointment with a new GI on 15/5 (I have moved countries since my last appointment).
 
I not sure my experience will help.

I had surgery for adhesions over 19 years ago. The pain from the adhesions followed my monthly cycle due to their location. My left Fallopian tube was join to the left ovary by adhesions. I had 6 hours of micro surgery which helped greatly.

My adhesions are of unknown origin probably due to an infection. I did not have Crohn's.
