Best way to treat fissures?

I keep having fissures due to chronic diarrhea which are causing irritation, itching and minor bleeding. Is there a recommended cream or treatment that seems to help?
 
Thanks I do use the creams with little help but maybe someone can recommend something that worked well for them.
 
I've tried everything besides surgery ...the only sure thing that will heal a fissure, if it is chronic, is getting your crohn's into remission. Over time it will heal, but if the inflammation is there from crohn's, it'll open right back up.

How bad is the pain for you?

For me in the beginning I had major fissures and shook on the toilet because of how painful it was, and it would be that painful for 5 minutes or more until endorphins and natural opiates kicked in. Thankfully now its just a minor irritation besides the new hemmroid sitting on top of one fissure.
 
i believe fissures are due to iron deficiency and vitamin a deficiency, and not so much directly related to diarrhea, but it is related to diarrhea for sure.
Iron is known to boost retinol binding proteins which enables retinol(vitamin a) to be delivered to tissues. vitamin a is very important regulate of epithelial(outermost skin) integrity, and this includes the intestine as well as the more well known skin on our body.

so it is indirectly related to chronic diarrhea. in crohns disease,disease mainly affects the ileum and this is where most if not all fat soluble vitamins are absorbed(a,d,e,k). With so much mucus being produced, the fat cannot be absorbed and therefore the vitamins cannot be absorbed either and escapes digestion and will manifest itself as floating fecal matter, because oil/fat floats upon the surface of water. if you arent absorbing your dietary fat, you aren't absorbing your fat soluble vitamins.

so it is most likely the fat soluble vitamin a, and second most likely iron intake that is involved in fissures, where the skin becomes thin, dry and loses its elasticity and then cracks and has a hard time healing, the anal orifice requires great amount of elasticity to function properly.
 
I woke up last night from the pain. My disease is not under control but hopefully Cimzia will help that.

How long till it heals if this medication works?

I am iron deficient but not terribly anemic since my last infusion so that shouldn't be the issue. I also get a lot of vitamin A in my supplements.
 
I Have several fissures in my rectum was bleeding quite bad. My Gi put me on Canasa 2 times a day after about 4 months of use I no longer bleed and it doesn't hurt or burn or anything if it starts to feel irritated I will use one. For me they work great.
 
Some of the over the counter products that helped me when I had this problem were Balneol which is a white creamy liquid used afterwards and Tucks medicated pads. I also found sitting in warm baths soothing.
 
I've had Botox treatment for a fissure almost 3 years ago. It didn't help unfortunately. Now my doctor put me on these "rectal rockets" that contained hydrocortisosone, carafate, lidocaine and cytotec. I did one each night for 4 nights in a row. After I had to start hydrocortisosone suppositories twice a day for 2 weeks. Been doing those for a week now and I think it's helped a little, but not enough. It's looking like I'll have to get the surgery to correct it. I'll be interested to see other replies to see if any other types of treatments have helped anyone else!
