intubation was successful. the procedure was tolerated well by the patient. the bowel preparation was satisfactory.the amount of bowel preparation used was 2sachets.The egzamination was completed. the instrument was inserted to the terminal ileum. This was90cm from the anus. The caecum was indentified by The Ileo-Caecal Valve; The Appendiceal orfice; The Tri-Radiate caecal fold. biopsy from whole colon 12 samples

It looks as if it is saying the colonoscopy went well the docs were able to get the scope into the terminal ileum which is the bottom end of the small bowel. The scope was advanced through the entire colon(large bowel), into the cecum(a pouch connected to the junction of the small and large intestines) which was identified by the ileocecal valve(valve between the ileum of the small intestine and the cecum of the large intestine), appendiceal orifice, and triradiate fold. So they were able to see into the terminal ileum of the small bowel(this is as far as a scope will usually see into the small bowel). It looks as if they took twelve biopsies during the colonoscopy which will be examined for various issues under microscope.
