Changing Jobs

So I've decided it's time for me to change jobs. After 8 years as an elementary music teacher, the last 2 with Crohns and migraines, I've had it. This past year has been too stressful, there has been too little support, there have been too many negative changes at the state level in the educational system, and I can also expect poorer working conditions in my school should I return in September.

I've been trying to look, but it's been a struggle, being wiped out physically and mentally with Lyme disease. I'm trying to get some help from a state department that handles some job placement stuff for people with disabilities, including some not-so-severe disabilities. One of my big worries has been how to handle going through interviews and such, with wanting to find out about flexibility, but not wanting to say too much about my health conditions. I prefer to be open and up-front, but it seems the going wisdom is not to breathe a word of having any sort of issue. But I don't want to wind up in a job that's going to be up my ass for being 5 minutes late because I needed a 2nd bathroom trip in the morning, or because I needed a few bathroom breaks during the day, and so forth. Starting a new job, only to have to change jobs again a month in isn't good.

Then there's insurance. Even if I manage to find something on my own, or this state department (who I will meet with for the first time next week--it took over a month from when I got ahold of them to have an appointment) approves me quickly and finds me something quickly, I can expect there to be a gap in insurance. I'm on 5 prescription meds, Humira most notably, that I can't just stop.

Does anybody have any advice on how to handle changing jobs and covering the insurance gap, possibly from personal experience?

Or even advice on what sort of work to look for? I'm somewhat limited by holding a BA in Music, and being poor at phone-based stuff, so customer service and sales are not good options for me. I've been thinking some kind of desk job, office environment. I know my way around a computer as well as copiers, faxes, etc., but lack any official certification.
