COLONOSCOPIC SURVEILLANCE AND MANAGEMENT OF DYSPLASIA IN INFLAMMATORY BOWEL DISEASE - Starts on page 48.
Introduction:
An increased risk of colorectal cancer (CRC) in inflammatory bowel disease (IBD) has been recognised for many years, though the best way to manage this problem depends on a number of individual co- factors. Once IBD-associated CRC becomes symptomatic, the malignancy is usually advanced, and prognosis correspondingly poor.1 Prophylactic proctocolectomy avoids CRC, but is not justified in patients whose health is otherwise good due to significant post operative morbidity.2 Colonoscopic surveillance has been advocated as an alternative means to reduce cancer-related mortality among individuals who wish to retain their colon. For the first time, these Guidelines have included specific recommendations regarding colonoscopic surveillance in IBD. Many of the conclusions are similar to those contained in recent updates on this subject issued by the American Gastroenterological Association3,4 and the British Society of Gastroenterology and Association of Coloproctology for Great Britain and Ireland.5
