Difference between IBD fistula and non

I developed a fistula in September and have been treated with flagyl And cipro on 4 different occasions. It temporarily closes but also reopenes. Just a couple weeks ago I have excruciating pain on my bottom. There were four lumps from my anus to tailbone. My colorectal drained one and said the abscess was infected and started my on flagyl and cipro again. The fistula is temporarily closed but ive developed two lumps again and that becoming more painful by the day. What can I do? How would they know if this was an IBD fistula or not?
 
My heart goes out to you. If you have a crohn's diagnosis it probably a crohn's complication . Does the doctor think it's something else?

Is there any discussion of further treatment with biological meds like Remicade?
 
There was inflammation but not definitive crohns. The doctors are unsure at this point. MRI was done and they saw sinus tracts but no end point. There has been discussion of remicade but they do not feel as though the inflammation is severe enough at this time.
