Question?

On 3rd in two days, I am having pre assestment apt for colonscope on 10th. I am inter piles which starting bleed today.
Does anyone if the hospital can use antiseptic cream or something for pain on pile?

Thank you.

I've not heard of someone having a pre-assessment for a colonoscopy before, have you been told why your having one? They can but I'm not sure they would give it during a pre assessment. It won't do any harm to ask them though. During the colonoscopy itself you should have some form of sedation and pain relief and they will top this up if you have discomfort.

Hi There

I work in an Endoscopy unit in London as an administrator. It is common practice to have a pre-assessment before most procedures.

It is not an invasive pre-assessment ie: no chest x-rays etc its mainly a questionnaire. This highlights is a patient is on certain medications and whether they need to be stopped prior to the procedure. If a patient is on Chemotherapy and if bloods need to be checked before the procedure. Any disabilities and sleep apnoa or past problems having sedation.

If you have Piles you should continue taking cream / suppositories and the Endoscopist can also use a cream to numb the area before scoping you. If you have further questions you should contact the Endoscopy unit for further advice. I hope this is helpful and your procedure goes well for you.

They are unlikely to be piles. They are more likely to be inflammatory tissue, particularly if you have inflammation of the colon and/or other perianal disease like fissures.

Generally they will give you sedation which should limit or cover up the pain altogether.

However I would be sure to mention your concern to the GI who is doing the procedure so they are gentle and take precautions to reduce your pain.

Hi Patricia,
I know I have inter piles because I took myself E&A earlier this year also I have suffer quite bad past and surgery band few years back.

I mentioned it because it is very important to be able to tell the difference.

Inflammatory tissue can mimic piles closely and only a GI is likely to be able to tell the difference. Most doctors will conclude they are piles despite a failure of the regular treatment for piles. And of course you may have both - complicating things immensely.

If the tissue improves when you are in remission or you have little pouches left behind after the swelling goes down then it is inflammatory and the treatment for piles will not be work. Only getting your CD into remission will do that.

In the meantime I would ask your doctor to order a compounded cream that includes nitroglycerine which you can apply to the area to numb it and which may have some therapeutic benefit.

Here's a link to an article about it.

I hope you are feeling better soon and get some relief from the pain.
