Fever as an Aza side effect?

Hi all,

Two weeks ago, I was placed on 50mg Azathioprine for the treatment of moderate/highly active Ulcerative Colitis. As someone who tends to be sensitive to strong medications (especially those affecting the stomach), I was pleasantly surprised that the first 6 days on Aza were side-effect free. However, on the 7th day, 3 hours after taking my dose in the afternoon, I experienced bad stomach cramps, vomiting, shakes and a subsequent high fever (38-39c), effects which vanished by the next morning.

After two days of these symptoms, I was prescribed anti-nausea tablets which eliminated the cramps, nausea and vomiting. Despite this, I still have painful muscle stiffness and tenderness in both arms and legs. Furthermore, while my temperature is normal throughout the day, it spikes during the night/early morning (2-4 am), last night reaching 39.6. Strangely, I experience no chills or shakes in this instance. Fever abates with a dose of paracetamol, but returns regardless the following night. In the morning, I'm left feeling 'flu-ey'.

My doctor is at a loss to explain this, as my bloods (including white cell and crp) are fine. He has suggested stopping the Aza until Monday, when he will have the results of a urine test, before potentially starting on a broad spectrum antibiotic. I'm just worried that if my fever stops, I'll have to stop Aza (I really don't want surgery), but if not, I'm going to be laid up with a succession of mystery conditions for the duration of my treatment.

Has anyone else here experienced something similar? I questioned whether I might be allergic to Aza, but my doctor dismissed the idea.

I didn't have any fevers, but I had extreme chills, nausea and vomiting while on it. I was told to stop it when I ended up in the hospital from dehydration.

I was on Azathioprine for 2 weeks and then one day around 11am i started feeling not well, sickly, and my fever shot up to 40c - as i live in Australia i also had the horrible timing of a 40c day - the fever persisted around the 39-40c most of the day. I tried having a cup of tea and a banana at lunch of which i threw up an hour later. the fever continued to go and i sipped cordial till about 5pm when severe vomiting began... I could not move i was bed ridden and helpless - I was also on 50mg of pred that usually masks fevers - by 12:30am i was admitted into hospital - severely dehydrated (as was unable to hold down any liquid at all and was dry reaching) they took me off all meds for 2 days and immediately felt better - the morning i started the Azathioprine 3hrs later my fever shot up and i was vomiting again. I refused to take the Azathioprine the next day and was released from hospital 2 days later as no further symptoms had occurred.

I hope my experience helps you - it does sound like you are slightly allergic to the Azathioprine
