Hello everyone,
Looking for some insight. I'm a healthy 42 year old male. I've had IBS type symptoms for over 10 years now since a cryptosporidium infection. My typical symptoms were some occasional episodes of diarrhea and cramps brought on by stress or certain foods. But about a month ago my situation changed. I had what I assumed at the time was a GI infection (a day of fever and nonbloody diarrhea). Felt better by the evening after taking (quite a bit) of Pepto Bismol. A few days later I awoke to an intense lower abdominal cramping and the urge to have a BM every 30 minutes or so. Sometimes I'd have a small BM, sometimes just gas (but no blood). These symptoms improved by the evening. But ever since then I've had pretty much constant, lower abdominal pains, lack of appetite and very loud / active gurgling at night that has caused me to lose a ton of sleep. I saw my GI who did stool studies for infection (negative), a CBC (negative). He also did a colonoscopy which he said showed "some small erosions in your colon and ileum that were not suggestive of Crohn's disease." The path report came back "focal, mildly active ileitis and colitis with essentially preserved architecture. Not typical of Crohn's, more likely infectious but IBD cannot be ruled out." My stools have returned to normal (soft, formed, brown and 2-3 per day) but I continue to struggle with abdominal pain, loss of appetite and trouble sleeping due to my gurgling and cramping. Sound like Crohn's? My biggest concern is those findings on pathology and my nighttime symptoms (which are causing me a lack of sleep which definitely isn't helping!). Anyone have any insight or advice?
Thanks!
Max

do you know if they ruled out Celiac? I would get a second opinion on the path report if possible...I'm not sure what essentially preserved architecture indicates, but ileitis is more often linked to Crohn's, unless it was infectious like you said. Yeah an MRI or CT might be helpful! Good luck, and remember to always advocate for yourself to the Dr's!

Quick update. Still having abdominal pain and occasionally waking up at night with "gurgling and churning". Loose stools 2-3 x per day (which for me was typical even before all this started) but have only had watery diarrhea 2 or 3 times in the last month. After talking with my GI they ordered CRP, ESR and fecal calprotectin. All were normal! With those tests being negative I feel Crohn's is less likely. Thanks for your replies and support.

Gosh, sounds like some of the more experienced members could chime in on this thread.
You should be receiving some sort of treatment.
What about getting a CT Scan ?
I am new to Crohn's.
Keep us updated.
Sending you my support. ❤

My fecal calprotectin was 27, my CRP was <0.03 even without any treatment. My biopsy showed "preserved mucosal architecture" and in Crohn's the architecture is distorted (granulomas, etc.). So my doctors think most likely the inflammation on colonoscopy was due to an infection and my symptoms will improve over time.

Hi. I have had similar symptoms since February. The gushing gurgling was constant followed by diarrhea 2 to 3 times per hour. I ended up being diagnosed with microscopic colitis after 3 months of suffering. Hope things get better for you.
