I use ice packs on my knees and elbows, you can get those gel wraps you keep in the freezer, barring that you can put ice in a baggie and put that inside a towel so it's not too cold on your skin. I don't know if that works for anyone else or if that's the "right thing" but that seems to work for me.
 
I have found heat helps and use a heating pad on my back and a hot water bottle on my aching tum.My doc thinks it is Crohn's arthritis and coinciding with a flare.
Today I tried therapeutic massage by a registered massage therapist.
She found all the achy joints and points over my body and treated with massage.
This evening I am a lot less achy so it may have worked better than anticipated.

If you are getting serious swelling, you really need to have the joint drained if possible. I have dealt with the very painful swelling in my right knee on and off for 5 years, and both my GI and Rheumatologist have said to get it drained to prevent joint destruction. The first couple years I did not know this and was not under the level of care that I am now, and I still have problems with my one knee even in remission.

If it's fluid that can be drained, you should be able to get this done at a walk-in, or at your PCP. If they won't do it, tell them to get you somewhere that will. Don't take no for an answer, and don't let them delay. Your health is more important than their convenience.
