I need help with this terminology

I recently had a colonoscopy that showed sever terminal ileum inflammation, right after the procedure the Dr said she was fairly sure it was crohns and that the tests will more than likely come back negative due to the nature of the inflammation. In the full lab report all the tests came back negative. It also used the words "dense lymphoplasmacytic inflammation" and ulceration in the terminal ileum. I was wanting to know if anyone knew what "dense lymphoplasmacytic inflammation" meant im scared its something worse than crohns.
 
Hello, I think it means inflammation to the surface of the mucosa??? But I am not sure.
Crohns can affect the whole thickness. Wish this was more help. I know waiting for results can be scary.

Its not so much waiting for the results anymore as its I have the results and they are all negative for crohns but when I had the colonoscopy the Dr said it was probably crohns. I'm a have an issue where when I read medical words I google them and I don't like what I find. So when I searched that tern it said possible lyphoma. So I was wanting to know if the test they ran would show lmyphoma or if ill need more tests. The picture of my terminal ileum shows it is pretty much all white with some red. My next step is finding a G.I. All this started with rib pain then since then I've had random pain in just about every bone in my body. And now I have regular knee and foot pain. Also lost 20lbs since this started but I changed my diet and added lots of vitamins. Dr said I have slightly small red blood cells and I was slightly vitamin d diffecent. I'm alright with having crohns I just don't want cancer lol
 
I also have daily lower left rib pain. All of this seemed to happen in one day so its all rather confusing to me. Xrays of chest show no rib issue. I'm just hoping someone else has had these same things and can say it is crohns. Thanks

"dense lymphoplasmacytic inflammation"

This is a descriptive term and is saying that there is a high volume of lymphocyte and plasma cells accumulating in the lining of the intestinal tract that is inflamed.

To put this into perspective of what is occurring in your bowel:

  • The area of the bowel that is affected by inflammation is the most common site for Crohn’s disease.
  • This area of the bowel has a large number of lymph nodes, plus the appendix, so it is not uncommon to see lymph node involvement to some extent and in the vast majority of cases there is nothing suspicious about it.
  • Lymphocytes are one type of white blood cell. They are the most abundant of the white cells and play a key role in the both the immune system and the inflammatory response. Therefore you would expect to see lymphocytes where there is inflammation.
  • Plasma cells are in fact a type of lymphocyte, so again you would expect them to be present when chronic inflammation is present.
  • By the sounds of the your terminal ileum then dense lymphoplasmacytic inflammation would not be an unusual finding on pathology.

Whilst biopsies are taken during a scope they can’t biopsy lymph nodes as they are located on the outside of the bowel. A CT scan or MRI will show if there is anything abnormal with the lymph nodes (lymphoma).

so what you're saying is that it is more than likely crohn's? and that lymphoplasmacytic inflammation is pretty common form of inflammation involved in crohns?

I’m not a doctor Prologue so please don’t take what I say as gospel.

Pathologists do use different terminology and whilst I wouldn’t say it is a term that is widely used I can see how it is used when describing inflammation of the small bowel.

I can’t say that what you have is Crohn’s but many of your presenting symptoms and many of the clinical findings…what the doctor visualised, the bloods, the location of the inflammation…mean that Crohn’s is right up there on the list of possibilities.

Since inflammation is present then it would not be unusual to have the affected area heavily infiltrated with this type of feature...lympho (lymphocte) plasma (plasma) cytic (cell).

The problem with obtaining a diagnosis of Crohn’s is the gold standard they look for in a biopsy is granulomas. The problem with this being that about 50% of Crohn’s sufferers do not have granulomas and it is these circumstances it is up the GI to make an educated call.
For example, my son’s scope back negative for Crohn’s and when he had his resection the pathology returned a finding of…no convincing evidence of Crohn’s. He certainly has Crohn’s, just doesn’t tick all the boxes when it comes to pathological evidence.

Good to know dustykat I really appreciate all of your help. As soon as I got out of the colonoscopy the Dr pretty much said that's what it would be and that more than likely it would come back negative.
 
Rib pain could be the gallbladder. An ultrasound can sometimes help in seeing if stones are present.
 
I wouldn't say I have much pain in my abdomen I do have pain there but I have all these other random aches and pains that bug me more. I'm thinking its due to vitamin deficiencys.
