Low Alkaline Phosphatase?

Anyone have experience with low alkaline phosphatase? Over the course of a year, DS's has been dropping. He's just barely in the normal range now. I read it can relate to malnutrition, but not sure how to interpret the dropping numbers.

Thoughts?
 
I found a rather long list which are mainly vitamin or mineral deficiencies, but also Vit D excess and hypothyroidism. Did he start or increase vit D during this time? Also, the decrease that you see might not be significant, but you can mention it to his doctor.

A decreased serum alkaline phosphatase may be due to:

Zinc deficiency.
Hypothyroidism.
Vitamin C deficiency/Scurvy.
Folic acid deficiency.
Excess Vitamin D intake.
Low phosphorus levels (hypophosphatasia)
Celiac disease.
Malnutrition with low protein assimilation (including low stomach acid production/hypochlorhydria).
Insufficient Parathyroid gland function.
Pernicious anemia
Vitamin B6 insufficiency

He did start vitamin D, about 600mg/day. That's not much tho, is it? And he has Celiac disease, but that is well under control. He was anemic this summer.

You would normally expect the ALP numbers to be within range or going the other way in an adolescent Mehita. They are often elevated, not below normal, as they increase with growth and puberty.

Since this is not the case you would have to think that active disease and malabsorption are likely playing a part in these decreasing numbers and it is something that the doctor needs to address.

My two have had ileal resections and whilst I am not up on where is at with her bloods and supplements Matt certainly has issues with Magnesium, Zinc, Vitamin D and B12. I will confess that I am not always happy with normal reference ranges when he is sitting at the lower end of them and so aim closer to upper limits and supplement accordingly.

Do you mean 600iu? If so, then yes that is a low dose. I had Matt on 2,000iu and a few months ago increased him to 4,000iu.

Yes, 600iu. Everything makes sense now with the confirmed flare, abscess, fistula, blah, blah, blah. Spending the week in the hospital to work it all out. Fun!
