Wow. I can't even imagine. I'm lucky enough to have a bathroom in my classroom and students can go whenever they want without asking. Some of the teachers in our school don't have their own bathrooms because they are in older wings, so students have to raise their hands to ask before they leave the classroom, but I've never heard of a teacher saying, "no" except when it happened to my nephew in high school. He repeatedly asked to leave during one class period and kept getting up for water. It was a huge distraction and finally the teacher refused to let him out of his seat again. When my nephew got up anyway he was sent to the principal. The principal called my brother who told him to call an ambulance right away because it was so out of character for my nephew and it sounded like he was going into diabetic shock. Mind you, no one suspected my nephew had diabetes at the time, but it does run in my sister-in-law's family, so my brother suspected it immediately. By the time the ambulance got to the school he was in real trouble, but luckily he made it to the hospital in time. Now he has to give himself insulin every day. I know the teacher was annoyed, but I'm surprised she didn't find it odd that a kid who had never been in trouble before was defiantly demanding water and the bathroom repeatedly. Maybe it's different in high school since they see so many students.
