Painful bowel movements!

Hi, I am 15 years old and diagnosed with Crohn's and I really need help. I have been experiencing painful bowel movements for the past year and it was originally why i had a colonoscopy resulting in my diagnosis. After that I was prescribed prednisone and I have been on a gluten free diet along with good pro biotic food. Even though I have been taking this medication my bowel movements have only worsened and I'm posting here because of my bowel movement from 2 days ago which was the worst I had in my life. I must have been on the toilet for over an hour in pain. The bowel movement itself was extremely painful yet really hard to pass at the same time. I had to strain a lot for each little piece but could only do so for short moments at a time because of the pain and even with this i could hardly ever get anything out. Each time I managed to make a bit poke out I had to scoop it away with toilet paper just to get it out. After the bowel movement my mom told me the outside of my anus was swollen but this swelling was gone in 30 minutes or so. From research done before this particular bowel movement I thought i might have proctitis but I was told that from the colonoscopy results that they didn't see any inflammation in the rectal tract. Could this be an anal fissure or something? All my bowel movements have been like this for a year except this last one was just so much worse. Please help, i fear my next bowel movement. :(

I also wanted to add that to remedy the pain after each bowel movement I have to sit on a heating pad for a few hours at least and that like an hour after each bowel movement there is a little bit of a yellowish discharge from my anus.
 
You really need to go back and see your GI. Or get a second opinion if you feel this particular one isn't being all that proactive (handing you Prednisone and that's it?). What dose did they prescribe? How long did you take it or are you still taking it? As you can tell it doesn't seem to be doing the job. When you see discharge that you know isn't mucus it could be a sign of infection from an abscess or even a fistula (and possibly something I'm forgetting). Either way you need some tests done and I agree a colonoscopy is the first one that comes to mind. You can't live your life like this no matter what your age. You're in pain for far too long and too much pain it sounds like. Let us know what your GI says. Have your mom call right away if she hasn't already.
 
Hi Klotrok, Ouch, it sounds so painful. When was your last colonoscopy? I am not sure if you have an anal fissure, but I would go to the GI doctor and see what he says. :( What medications are you on now?

My colonoscopy was done a month ago and I already did have bowel movements like these at the time but they said no inflammation was seen in the rectal tract which made me think an anal fissure was possible because i heard they can be hard to see. I have been taking two 20 mg tablets of prednisone a day for the past 3 weeks or so and I start tapering down in a few days. It's hard to say if prednisone is helping me or not because I was told by a doctor that this current rectal pain problem might be unrelated to the inflammation in my digestive tract from Crohn's and whatever stomach pain i do get is possibly from not having a bowel movement for too long. Isn't pred used for inflammation and therefore could have no effect on whatever the cause of this rectal pain is? I also started taking low dose naltrexone 2 days ago as prescribed by one of my doctors who is a professor at a university of natural medicine because he said it's a good alternative to keeping me in remission than what is normally prescribed and has more side effects. I had my first MRI done yesterday so i'll find out soon if pred is working and based on research done about anal fissure I found that people said doing sitz baths with epsom salt helps so I'm trying that now.

So sorry!
A colorectal surgeon prescribed NITRO-BID for my son to keep the muscles in the area relaxed. He uses it daily at bedtime. He is not pain free, but it definitely helps.
