Hi.

I got my pathology report from my last colonoscopy. As far as what I can read it sounds like he should be treating me for crohns'
Please tell me what you all think???

Sections show multiple fragments of the terminal ileum. The surgace is focally ulcerated and shows multiple areas of intraepithlial neutrophil infiltration. The lamina propria is markedly expanded by enlarged lymphoid aggregates. A small focucl of neutrophilis aggregation is also noted. The crypt architecture is mainly intact. No crypt absecess formation is indentified. no granuloma is present . There is no evidence of dysplasia or malignancy.
Diagnosis : Acute ieitis with focal surgace ulcertation and extensive lymphoid aggregate formation. Differential diagnosis including infectious vs early inflammatory bowel disease, clinical correlation is recommended.

Can any of you make sense of this.

Hi,
It sounds like early stage Crohns of the Ileum. The report can't confirm Crohns from a visual perspective only. It says 'differential diagnosis' (which means 'possible causes') include early IBD. No granuloma or crypt abscesses shows early disease only. The neutrophil collection (infiltration) in the crypts can eventually do more damage. The report recommends correlation of all the other information that is available (like blood tests etc) to make a proper diagnosis.
Good luck, maybe it's in the early stages and can be managed ok?
 
It might not be at the point to be treating for crohns just yet, the ulcers haven't done the damage to cause concern. He also might feel that the possibility that these are done by infection and may treat for infection instead.

I was treated for infection for a year(the ER would treat for basic infection instead of looking further) I got treated with 3 different rounds of antibiotics before I had lost so much blood I was admitted to the hospital. Don't get to frustraited it looks like your on the right path(and now you know it's not "all in your head") Your one step closer to getting a dignosis

The only thing is I have such pains and diarrhea, that right now I am taking Lomotil once a day just to get by and I think I might have to increase to 2 a day, seems these past few days the lomotil is not quite doing the same as it use to.
