Please help my research! Survey on Personal Health Records and IBD

Hi all,

I'm a graduate student at Carnegie Mellon University in Pittsburgh, PA and a fellow IBD patient. For my master's thesis I am working to design a tool to support the self-management of IBD. Please take 5 to 10 minutes to complete the following survey: http://www.bit.ly/nGmumd

Absolutely no personal information is collected from participants (unless volunteered). Participants can answer as many or as few questions as they are comfortable with. Additionally, this survey has been vetted by Carnegie Mellon's Office of Research Integrity and Compliance.

Your replies would be greatly appreciated! When my project is finished, I will share what I learn and design with members of the Crohn's Forum community.

It took about 45 min but I remembered some stuff later lol. So I sent her an email. She is very nice and professional. You are probably about her age, but not sure. So glad you are participating, and ya know it is for OUR benefit.

Thank you to everyone for your help and support! Especially to Pen and Emily for further assisting me with my interviews, and of course to David for allowing me to post. This is a great community (wish there was one this strong for UC!) and I'm so thankful for the support of the IBD community as a whole. We're all in this together, after all.
