Ulcers on stoma

Hi all, my crohns was diagnosed in my large intenstine which I had an emergency colectomy in november 2010 leaving just a rectal stump (which gives me nothing but problems) however since being on weekly humira and 6mp it has started 2calm down and was finally looking into a reversal, now the last few days ive had the most painful skin ulcer about 1cm away from my bum (not nice) and ive noticed tiny little ulcers o my stoma. Has anyone else had this problem? And does anyone know if this means the crohns has now started to affect my small bowel? Thanks for any input...
 
Ouch! It may not be your crohns coming back first of all. As we are immunosuppressed, it may be an infection. It is definetely something to not wait around to see the Dr about though. Especially as you are wanting a reversal at some stage. First, you should ring your IBD nurse or Stoma nurse and let them have a good look. They should be able to get you into the GI quicker that way.

Do you have any other symptoms to make you think it's a flare?

Thanks for your reply. Yea I do think its a flare again as I have a mouth full of ulcers, bleeding quite a bit from rectal stump, skin ulcers. All these symptoms are normal 2me during a flare only now got the ulcers on stoma and a grumpy belly :( I see my consultant on monday luckly who I have 2say is fantastic. Id just b quite gutted if I now find out its affectin my small bowel 2. I will keep u posted after my appointment, hopefully its just a infection... Thanks again for replying

Hi Molly.
It would be great to hear what the outcome of your doc visit is. Skin and mouth ulcerations are also a feature of my CD. I'm about to start Humira as my control medication. In the interim though, I have had a horribly ulcerated mouth, had an ulceration on my labia and I am worried abut what looks like an ulceration at the top of where my stoma is stitched to my tummy. I have a CRS & stoma nurse visit tomorrow - so will be having them all checked out. I will let you know what they say.
 
Hi samboi, thanks for your reply. Will let u know the outcome of my drs visit tomorrow, would also be keen 2know how ur visit goes also. Im glad 2hear u are starting humira as it has really changed my quality of life, what I found at first it wasnt lasting the full 2weeks so am now on weekly and it realy works for me, well up 2this latest flare anyway. Good luck tomorrow and keep us posted.
 
Saw CRS & Stoma Nurse today. Both were clear that the ulcerations are a sign of active disease ... which has been exacerbated by - the ulcerations! Mouth ulcers are preventing the nutrition I need to eat to help heal the ulcers everywhere else. Once the disease is back under control, the mouth ulcers will stop, the nutrition will get through and make me healthier, ulcers will recede. The stoma nurse was actually pretty happy with how the stoma is looking - given how unwell I have been.
No mouth ulcers today - so I will eat like a beast!
Really looking forward to starting Humira so I can get this all back under control.
 
Hi guys, firstly samboi glad ur getting the advice and treatment u need. Like u ive been told its anothet flare and my consultant is just as gutted as me that its starting in small bowel now aswell. Been given a course of the dreaded steriods :( and up my dosage of 6mp and if that dont wirk hes going to put my humira up to twice weekly. Thanks 2 u both for advice. Xxxx

I have a loop colostomy and am awaiting the removal of the defunctioned bowel and some other bits of my small and large bowel. My surgeons are not sure whether I'll be left with a bag, joined back up or something inbetween like a bag and rectal stump and I just wondered what problems you have from yours?

I also have ulcers on my stoma which come and go and I have ones just inside it too, which is my Crohn's. Are you on 6mp and humira? Do you mind me asking what your diet's like? It's just that I've found in the past for me that resting my bowel and having low residue/elemental diet has helped me far more than increasing those kind of medicines.

Hi Angela, thanks for your response. the problems I still have from the rectal stump is a lot of bleeding and can give bad abdominal pains, the ulcers on stoma are a new thing for me which have cleared uo since being on the steroids this week. My diet is fairly balanced I avoid my trigger foods as much as pissible which for me is tomatoes, pineapple, cheese and chocolate. Im also a light smoker abiut 5per day which probably doesnt help and I do enjoy the odd drink. Xx
 
Has your specialist brought up the possibility of the ulceration being pyroderma gangrenosum? It is a Crohns related condition that often appears around stoma, it has quite a distinctive look to it so your doc should know straight away if it is this. Can be treated with steroid injections into the area which clear it up pretty quickly.
