Cutting seton or fistulotomy?

I have a low, intersphincteric anal fistula and my options for surgery are (1) cutting seton (2) fistulotomy. I would appreciate feedback regarding these 2 options - i.e. success rate, involvement of the sphincter muscle, etc. Thanks in advance.

I believe the cutting seton is the best option. They work. I have had one in the same area. The fistulotomy can cause permanent continence issues depending on how much muscle is lost, not good.

Thanks, cmack - I can always count on you for good advice. Approx. how long does it take to cut through the muscle. My draining seton has caused a raw area by the anus where the string's knot sits & constantly rubs in that area. Not sure I could handle another seton for too long a time. Seeing my c/r surgeon in an hour to make a decision.

It really depends how long for it to cut through. It depends how much muscle is involved. It will likely take several months. I hope things go well.
