Hi all, long time no talk. We moved and things have been crazy around here. DD had an MRE a few months ago and that looked great and her labs have been great too (yay!). DS was in the hospital overnight a few months ago with what may have just been a stomach bug (because after he was sick we all got sick!). Then he twisted his ankle so badly the docs thought it was broken (thank God it wasn't). His ESR and CRP were a bit elevated at his infusion in June but I think maybe that was because of his ankle.

He had his most recent Remicade infusion yesterday and his albumin went up a bit and his ESR and CRP came down a bit so that's great. However, his SGOT was 157 (normal is up to 45)and SGPT was 100(normal says up to 48). I'm wondering what that could be and if I should be concerned? GI says to check again in two weeks but I'm wondering if I should be doing something before then?

Yes as I said I'm concerned but not sure what the values mean. He had suggested a protein supplement for him to gain weight last time he saw him but we never started him on it. He assumed we did because he told me to take him off the supplement and recheck in two weeks. Since nothing has changed except his liver values, I'm definitely concerned but not sure what the values mean (especially with his protein going up, inflammatory markers going down and all other labs were fine).
 
Can you call tomorrow and make sure the GI knows you ever started him on the supplements ?
That really important for the GI the know asap
 
Is he on MTX too?

Definitely tell your doc that your son is NOT on the protein supplement. That might change his view on the high labs.

We have had high labs and retested in two weeks, but ours weren't ever that high.
 
I called the on call GI and had a nice talk with him. DS hasn't been taking any supplements but has been weight training with his friend who does MMA. He says it's possible that it may be muscle breakdown because he's a thin kid who has never trained and is now doing an extensive workout. I also asked DS (who is 18) if he's had anything to drink in the last few days and he said he may have had 3 beers in the last week or so (my mother in law was in town and allowed it UGH). He's been on the same 5mg per kilo dose of Remicade for three years so he doesn't think it's that. I'm hopeful that the combination of him training too hard and having a few drinks was what threw it off. In the meantime, every other test looks great but I'm still going to retest in 1 week instead of two. Fingers crossed!!
 
Has the GI talked to him about drinking and IBD? Not so good with IBD and definitely not good on certain medications (like MTX). It's generally less of an issue with Remicade (or biologics in general) but it probably depends on the person.

My younger daughter's GI talked to her before she went off to college. She does not drink at all.

My older daughter was a different story. She has been on MTX and Humira for years, but that certainly did not stop her! She doesn't have IBD, but you are not supposed to drink at all on MTX. She would only drink on the weekend - at parties, when she went out with her friends etc. On college campuses, it doesn't matter that you're not 21...

Anyway, her rheumatologist did talk to her about it, and she did cut down. Luckily, her liver enzymes were never impacted by drinking. Now that she is older and the novelty has worn off, she is much better about it. Plus she graduated from college, so she isn't going to parties every weekend.

But it's definitely something to discuss with your son, now that he is 18. And it's something he should discuss with his GI because drinking can really be an issue for some people with IBD.
