Please can someone explain this colonoscopy result?

Would really appreciate it if someone could explain results to me?
My husband has a colonoscopy today. He has been very unwell with a perianal abcess that hasn't healed. An mri showed complex fistulas. We have had an awful time and he has been in hospital 4 times in the past 6 wks.

It reads

Perianal fistulae, which is discharging serous fluid. Moderately severe endoluminal crohns involving rectosigmoid colon. In addition caecum is grossly distorted due to chronic inflammation. Unable to intubate TI. Will arrange for infliximab asap

Perianal fistulae - connects the perianal skin with the anus or the rectum. Characterized by chronic infection and ulceration of the tissues around the anus. There are often deeply infiltrating, fistulous tracts which run under the skin and up to three to four inches along the rectal wall.

Fistulae - a narrow passage or duct formed by disease or injury, as one leading from an abscess to a free surface, or from one cavity to another.

Serous Fluid - the term serous fluid is used for various bodily fluids that are typically pale yellow and transparent, and of a benign nature, that fill the inside of body cavities. A common trait of serous fluids is their role in assisting digestion, excretion, and respiration.

Endoluminal - Edno meaning - inside/within; luminal/lumen - the canal, duct, or cavity of a tubular organ.

Rectosigmoid - The junction of the rectum and the sigmoid colon.

Caecum - Where the large intestine begins.

Intubate - To insert a tube.

TI - Terminal Ileum.

Infliximab - a monoclonal antibody (trade name Remicade) used to treat Crohn's disease and rheumatoid arthritis; administered by infusion; use and dosage must be determined by a physician.


LOL, wow that took a while. So what I'm getting from it is it means:
He has a fistulae near his anus (in the colon) and is leaking digestion fluid. They think it's Moderate/severe Crohn's Disease in the colon. There is a lot of inflammation especially in the ceacum. They were unable to fit the camera through the terminal ileum so they don't know what his terminal ileum looks like (it connects the large intestine to small intestine). They are going to treat him with Remicade.

I hope I did that right. :p All I did was translate the words used.

Edit: You can always ask the doctor or even sometimes the nurses know the lingo by now and can tell you what you want to know. All you have to do is call and ask (in case you haven't tried that yet :p ).

Very Good Crabby!!! I pondered on this, this morning but I had to go out for a bit , and then poof you came back with an amazing answer!! You clarified it better for me too!

Just to add, seeing all this mess, Remicade is great drug, and this is only my opinion but I think that is alot for Remicade to fix. Leaking anywhere can cause perentonitis, which is not good. Sounds like you and your husband may looking to a resection. He is in a lot of pain for sure. Blockages are so painful, having a baby is a piece of cake in comparison!!! One of my biggest fears..

My first resection done laproscopically was almost the same reasons for this result.

Let us know what you are both deciding. Surgery is supposed to be a last resort but not in all cases. Good luck!
 
Way to go, Crabby!! I couldn't have defined it better myself!! Sheesh! Maybe YOU can have the RN degree and I'll go do something else for a bit!! LOL!

Banrock: Remicade is a pretty awesome treatment for what he's got going on... it will either work right away and work wonders, or it won't, and he may need surgery. Either way, it sounds like whoever did the scope is very much on the ball, and going to get your hubby fixed up "ASAP" (as soon as possible, if not sooner...lol)

silvermoon - we are preparing ourselves that it may not work but have everything crossed it will. i honestly dont think things could be any worse than they are right now. thank you so much for kind thoughts and hugs, really needed them!!! xxx

Just as an FYI, I can understand pretty well what your husband is going through, as I am dealing with pretty much the same issues right now, regarding the perianal fistulae. While I was on Remicade it worked EXTREMELY well for me, but unfortunately, after about 5 years, it stopped working.... (just remember this doesn't happen with everyone...with some people the Remi can clear things right up and they don't have to worry for quite a while :) ).

Even if surgery is required, I have been there, done that too, and am actually going again this fall...meeting with the surgeons in Sept to set dates and plans.... Surgery sucks, but the QUALITY of life after is 1000000000% better.....
