Precertification for iron infusion???

In 2014 I had at least eight iron infusions which my health insurance covered 100% with no problem. Just got EOB for my first iron infusion in 2015 (wow -- didn't need one until November, so that part is cool). Now they aren't covering the cost ($1,600) because they say I failed to get pre-certification.. and of course I had another one two weeks later, (before getting the denial on the first one) so expecting they will say the same on that one... ouch $3,200 unexpectedly unpaid.

anyone else have to get pre-certified for an infusion? (this is in the US)
 
I had two infusions either last year or the year before and don't remember having to have precertificstion.
 
I had 3 iron infusions last month and I didn't need pre-certification. My infusions were pricey as well, about $1000 apiece before insurance (out of pocket, I'm paying $124 per infusion after insurance).
