Here is a link to a questionnaire that allows you to rate the degree of severity and frequency of symptoms associated with GI disorders. It does not cover IBD but nonetheless the questions look as though they may well be useful for parents in tracking the symptoms their children are experiencing. I don't think it would be too difficult to tailor it your own needs by adding things like blood in bowel motions etc.
