I told everyone I'd come back and post when I got a diagnosis for this ongoing skin issue I've had since last year - so here I am! There are sooo many new people here, hopefully this will help someone.

To recap - I started having hives around this time last year, which turned into an itchy, red, irritating rash that has moved around from my chest and back up to my neck and face and scalp. It looks sort of like acne, and sort of not like acne, kinda hard to describe. I have been sent to an allergist (who said I had rosacea), to my regular dermatologist twice (who said it was my Remicade and to get off of it. Eh...no thank you, I NEED that medication!) and to a rheumatologist so my GI could get the OK for me to keep taking the Remicade. They were concerned that maybe it might have been lupus or DILE (drug induced lupus erythematosus). Rheumy cleared me to continue Remi but uncovered another abnormal blood test (yay!! pile it on!). My GI said it was time to go to an immunological dermatologist at this point to try and get it resolved because she said it may just be Crohn's related skin issues. So - off I go.

The resident read my history and gave me a good look over. She then returned with the derm doc who immediately said she thinks it is melassezia folliculitis, or basically, a yeast overgrowth on my skin that causes this irritation. I wasn't buying it until she gave me the print out, and it mentioned use of sunscreen which is exactly when the rash broke out in a massive way, and the rest of it sort of fit. They took a punch biopsy (ouch!) and she said for me to bath with Selsun Blue shampoo all over until the results come back (ahhh - I get the spa treatment!). If they are positive, she will put me on a week's worth of ketoconazole pills, and that should fix me up! If it isn't that and she can't figure it out - then just shoot me, I give up!! And needless to say - I won't return to my original derm - she missed it twice even after putting eyeballs on me because she was very stuck on the Remicade related diagnosis. She never even mentioned the possibility of it being this.

Another side note for anyone finding this who is experiencing skin issues. In December I was on a short round of steroids for my Crohn's and it really seemed to help the rash on my back and chest a tremendous amount. Didn't make it go completely away - but helped with the redness and itching. But...it returned within a few weeks of being off the steroids. My first derm also gave me Cutivate lotion for the itching which is a steroid based lotion. The new derm said that while these steroid things actually help with the redness, inflammation and itching - in the long run they will actually make this condition worse because it sets up for a continued overgrowth of the yeast. I'd have to agree because the lotion seems to have made my face MUCH worse instead of better. Soooo...if you are trying to combat a skin issue with steroid stuff and it doesn't seem to be working - ask questions. Clindamycin solution (antibiotic) didn't do the trick either.

I'll report back once the biopsy results are back. Until then, I'm off to scrub up with my Selsun Blue!
 
Hey Peaches!!!!!!! So glad you are back! You have had it bad for awhile. When I got off the prednisone I started to get a rash on the back of my hands (I am allergic to sumac) and it was a windy day and thought when I was using grass seed, and had gloves on, but like a mesh on the back of my hands, I thought it was a reaction. Well it wasnt , but I use Hydrocortisone on it and Aveeno and it went away, as soon as that clears I got it on my under arms, but a different type , looked like Yeast and I have had it before. I had a cream which was used for herpes but ran out of it, so I comprimised with the hydrocortisone. I hate running to my doc for every little thing, he will thing I brought it on myself. I have a Gi appt this coming MOnday and have documented everything. Still on Methotrexate which lowers the immune system so I dunno.

Hope you get some relief soon and please DONT be a stranger, alot of people missed you on here, I know I did! ((Hugs))
 
Glad to hear you may be closer to figuring things out, Shantel! Enjoy your Selsun Blue bath! ;) Can't wait to hear the results of the biopsy.
 
I would say they can be sore rather than actually painful. But predominantly itchy. Here is the article that my derm printed out for me. There are a few pics there which look very similar to what I have. The sweaty part of the description doesn't fit, I'm just not a very sweaty person. But the sunscreen super fits, and my skin is a bit oily, even at my age. I also have dermatographia - so I think I get inflammation going on my skin a bunch easier than most people - and in turn, this causes more itching and redness.

Hey Peaches. You have been sorely missed here, but wish you were'nt having so much of a problem. I'm sitting here at my son's house holding my g-daughter Peaches and I find this post. Keep us informed on how things go.

I am glad you found a likely culprit for your worsening skin condition. I do not wear sun screen as a rule, but simply wear clothing to block the sun if i do not want exposure.

My Psoriasis suddenly got much better from laying directly in the hot Florida sun.
It is the only thing that made a difference in it. I wish I knew why that happened, but I don't.

Now I am back in MN, and it is snowing. It is depressing.

I had a rash like that when I was twenty. It was itchy, red and had sore bumps under the skin and little pimples. The dermatologist said it was rosacea. It spread to cover my face, scalp and neck and then after a year it began to clear. All I did was put a humidifier in the house and at work because the dermatologist said dry heat makes it worse. That was over 20 years ago. I never completely got rid of it but over the years it's been so mild that you can't notice it unless you look really hard and it doesn't bother me at all. I keep my skin cool and put vaseline on it if it breaks out. Now that I have been diagnosed with Crohn's I wonder if there's a link between rosacea and IBD.

Thanks for that info Gail. I don't *think* that it is rosacea - but I've been run around so much to different docs at this point, anything is possible. Hopefully the pill treatment they are thinking about is going to work. The Selsun Blue bath has made me pretty darn itchy and well, it *stinks*!! I'd do it for a month straight if it would get rid of this thing though LOL!

@Dan - my Mom has pustular psoriasis and in researching that a while back, phototherapy seemed to be one of the treatments - maybe this was what the Florida sun did for you? What better way to have a medical excuse to get out of that Minnesota snow from time to time :O) I can't believe it is snowing *now* - weird!!

Hopefully the biopsy will be back Monday and I can have the final answer ::fingers crossed::!!
 
Hi Peaches - Glad you are close to getting this thing figured out. It must be driving you MAD to have pain and itching like that.

Thanks for sharing your story and the info on the dx - I'm filing it away for future reference as we never know what we Crohnie's may run into in the future.

My mom had awful palmar plantar pustular psoriasis...Covered her feet on the bottom and was so painful she couldn't walk for months. The treament that finally worked was ultraviolet light...she would go to the doc's office and toast her feet and it's cleared up almost 100% now.
 
You are the second person that has recommended UV light, from first hand experience. I just happen to know where to get such a device. Trouble is I just spent $3,000.00 on a different weird device that works, for my wife's treatment.

I will add it to my list of weird treatments that work, and look into it more.
It makes sense then that the Sun improved it so quickly.

I've been dealing with a slew of skin issues lately - well, started about 2 years ago (Plantar-palmar pustular psoriasis to start).....that has cleared up, finally with some heavy-duty steroid cream.......had strep the end of March then really broke out - derm and I think it is a type of psoriasis (gutatte), but the biopsies came back NEGATIVE!.....the cream is helping, but if I miss a couple days it comes back.....ugh.....thin scaly skin, red circles, UGH!.....did I say that already?

If anyone wants to compare pictures, I think I still have some! :)
 
Pasobuff, I had a serious problem with red scaly circles about 10 years ago, and a dermatologist could never figure out what it was?! I still get a few of the circles each year, but not all over like I used to get. UGH is right!

Well, it's official from the biopsy. It is melassezia folliculitis - yay!! I mean, at least she figured it out. I have been on antifungals since Saturday and it does seem to be helping. Still doing the Selsun Blue as well (stinky stuff!). I asked her about sunscreen as this seems to be what started that whole thing off possibly, and she recommended a brand called Blue Lizard that only has zinc and titanium in it. She said if I wanted to spend more money I could buy Clinique's City Block - again - the one that is made from zinc and titanium without all those other chemicals in it that could cause an allergic reaction. Basically, stick with sensitive skin stuff.

I do have to get a liver function test after the pills are done as this is one of those meds that can mess with your liver, but hopefully that will all be fine. AND...I'm not itching much anymore - Y.A.Y!!

Shantel - I read about mineral sunscreens in Natural Health magazine and they recommended one by Alba Botanica (www.albabotanica.com) and it might be similar to the Blue Lizard sunscreens. It says it's chemical, phthalate, and oxybenzone free. And it's certified organic with the 2 main ingredients being zinc and titanium dioxide. Could be worth the ingredients comparison because it doesn't seem as expensive. ;) Just thought I'd throw that out there.

Would like to add a thumbs up for Alba. I use their sunscreen (and other products) and it's one of the few sunscreens that I'm not allergic to. Very easy on the skin, in my experience, although I am certainly not having any issues like your current problem.
