Role of Magnetic Resonance Enterography in Differentiating between Fibrotic and Active Inflammatory Small Bowel Stenosis in Patients with Crohn's Disease

ABSTRACT

Objective:
To assess the diagnostic accuracy of magnetic resonance imaging (MRI) in prospectively differentiating between fibrotic and active inflammatory small bowel stenosis in patients with Crohn's disease (CD).

Materials and Methods:
A total of 111 patients with histologically proven CD presenting with clinical and plain radiographic signs of small bowel obstruction underwent coronal and axial MRI scans after oral administration of polyethylene glycol solution. A stenosis was judged present if a small bowel segment had >80% lumen reduction as compared to an adjacent normal loop and mural thickening of >3 mm. At the level of the stenosis, both T2 signal intensity and post-gadolinium T1 enhancement were quantified using a 5-point scale (0: very low; 1: low; 2: moderate; 3: high; and 4: very high). A stenosis was considered fibrotic if the sum of the two values (activity score: AS) did not exceed 1.

Results:
A small bowel stenosis was identified in 48 out of 111 patients. Fibrosis was confirmed at histology in all of the 23 patients with AS of 0 or 1, who underwent surgery within 3 days of the MRI examination. In the remaining 25 patients (AS: 2–8), an active inflammatory stenosis was suspected and remission of the obstructive symptoms was obtained by means of medical treatment. One of these patients (AS: 2), however, underwent surgery after 14 days, due to recurrence. MRI had 95.8% sensitivity, 100% specificity, and 97.9% accuracy in the diagnosis of fibrotic stenosis.

Conclusion:
MRI is reliable in differentiating fibrotic from inflammatory small bowel stenosis in CD.

Thanks for posting, Dusty!! Has anyone ever had this test? When I had an obstruction last month they did a small bowel follow through to determine if it was due to active inflammation or a stricture, and decided that it was due to active inflammation....but I still wonder about how accurate that test is.
 
There are others here that have had this test and it is superior to a SBFT.

My son has had a CT enterogram, essentially the same test but a different scanner. It was very accurate at diagnosing the type and extent of his disease.

I had one done on Friday last week, awaiting results. Next Thursday 19th April is D Day!!

I had to drink a Ltr of stuff, All I can say is it tasted like metal and made me want to vomit. I had to run to toilet after i finished taking it before the scan and I had the worse D ever. They then gave me a shot of Buscapan to stop my bowels from moving. They put a canuala in and then they did they scan. Half way through I had a contrast into the canuala (this was not the same as the CT Scan one).

I will report back next week.
 
I am having an MRE done. How do they monitor if you are allergic to the contrast if you are in the machine. They can't see you.
 
I just had one last thursday, here's my experience in a nutshell: I was given a thin flavored barium liquid to drink (recommend drinking it chilled, but the taste wasn't too bad). I suggest bringing a book, portable DVD/MP3 player or something to keep you distracted and occupied. The MRI room was cold so you might want to bring warm socks and/or sweat pants to wear. I was laid on my back given an IV and told to put my arms above my head. The facility's MRI machine gave the tech good visability to see my chest rise/fall for breath holds and we were able communicate during the procedure. I was also given a call button in case of emergency. I had diarrhea an hour or so later, with abdominal pain but it wasn't anything worse that what I typically experience.

My advise would be to call the MRI dept to discuss your concerns and review the ingredients of the contrast to verify that the contrast is safe for you. Good luck and let us know how it goes!
 
I had this test done and it was fine. I this is a new test for me ever and it is great they have developed this for small bowel disease.

Had the test done. 90 minutes of drinking oral contrast then they give injected contrast and I was in the machine for 30 min. So much easier than a SBFT and no radiation that comes with a ct or X-ray.
