Any piaderma remedy advice?

Hello,

I have a case of piaderma proximate to my stoma. My doctors and I have tried oral steroids and steroid injections around the site, but the piaderma hasn't cleared up. I'd love *any* advice from folks who have had success with piaderma treatments.

Is pyoderma not an infection? Have you tried antibiotics? And how big of a patch is it? Can you cover it with a gauze pad with polysporin on it then put your flange over it to try and heal it up? That's what I did with the abscess I had beside my stoma and it worked like a charm. Sorry this hasn't been more helpful...hope it clears up for you!
 
Thanks for your note! I have tried multiple courses of antibiotics, as well. I haven't yet tried your polysporin trick, though have "packed" it with various substances (Hydrofera Blue, gauze, etc.). Typically, I cover the sores with a skin protective shield (tacky on the skin-side), which the flange sticks fairly well to.

Thanks again for your note. It is so encouraging to be able to bounce ideas off of folks who have a pretty good idea what I'm going through. I truly appreciate it.

Ok, first, I've never had pyoderma so what I'll be suggesting you what I have seen through time in the literature. Just to make sure, do you know if it is pyoderma gangrenosum, like sores that are ulcers like? If not, well ignore anything I may suggest here as this is what I am referring to.

It's somewhat a auto-immune correlated manifestation. I don't know what medication you are currently on, if any. Many of the anti-tnf used for crohn and UC have shown good results as well on these skin issues. I guess it depends of the severity of the skin condition. Usually it seems they keep one antibiotic and the medication they may use, either topical or systemic seems wide, ranging to specifics antibiotics to some chemo. I know there is a topical tacrolimus cream I don't know if that could be used there considering it's at the stoma site, I suppose your wafer goes on top?

I know that I'Ve seen the indication for pyoderma on my dressing and I know that it is suited for others. Dressing such with Silver in them can be helpful as it prevents infection and keep the wound moist for proper healing, you can see with your doctor if dressing like Mepilex AG or hydrocolloïdal could be suited such as Duoderm CGF.

I don't know if that will of any help but it probably worth knowing about these.
