This has been a huge question on my mind these days before any meal. I was wonder how you took notes and found out what upset your stomach? Is there some specific way to take notes of the things you eat and correlate it with stomache aches and D?

I don't think there's anything specific, just document everything in a journal.
Colour coding works well tho, ie red for pain etc, and putting the times too when pain comes.
Takes a while to get your head round transit times tho! Like what you've eaten prior could be giving you pain not what you've just eaten!

Also remember that food takes time to go through u whereas drink is pretty much instantaneous.

Have you tried an elimination diet? So cutting back everything except what you know doesn't affect you, and then adding one item in at a time. If you start to feel dodgy again you know what it was.
A hospital in the UK, called Addenbrookes, has done a load of research into crohn's and elimination diets. I believe that they use this method for a lot of their patients.

Keeping a food diary is helpful as long as you keep good notes and analyze it appropriately. I've kept a food diary for over a year (yes, documenting everything I eat and drink and when - and also documenting when pain or nausea occurs).

Through close analysis, I now have some idea of which foods cause which symptoms, but it's not an exact science. For example, I've found that dairy in the form of milk and certain cheeses causes gas; yogurt causes pain; solid chocolate causes pain, but chocolate made into chocolate milk does not cause any symptoms- which is weird because milk alone normally causes gas. Also, I can sometimes tolerate raw lettuce, but can't at other times - I have yet to figure out why.
