Hi
I have my apt to discuss my op soon which i dont mind after all the pain an hassle recently . Can someone let me know before hand what i can expect from a resection i have 15cm of the ilieum with issues and the main question i have is does surgery need a colostomy afterwards and for how long? Funnily this is my main worry the op i am ok with as i know this iwll help

Hi,
I am sorry to hear that you are going under the knife soon.

I had 10 inches (25 .4 cm) removed and no colostomy, but I think some people on here have mentioned having one when having less removed so perhaps it depends on who the surgeon is and other factors?

At any rate good luck with your surgery. I wish you a speedy recovery. :)

No I did not have a colostomy after my surgery. I had ~8 inches removed of my TI.

If your looking at losing a big chunk of your ilium removed, they might give you an iliostomy, not a colostomy.

Please don't take this as condescending as I just learned this yesterday when I was in seeing my GI. He threw the word at me and I about fell over. I'd been so worried about my colon having issues for so long as the thought of a colostomy freaks me out to no end, as it obviously does with you two.

My GI is a fabulous man! He spent ten minutes of my appointment spending time explaining the pill cam to my daughter who didn't know they existed. He brought out a sample and we discussed how it works, what the protocol is and that if it gets stuck in there it's usually just stuck, not like lit corks your intestines like a bottle.

He then mentioned iliostomy and that it will be up to my surgeon and what he thinks when he gets in there. He's actually spending this time between appointments trying to find the best surgeon for me (my old surgeon had an accident when he was on vacation and is a quadriplegic - therefore not able to work on me any longer), who will spend the time working this all out before he goes cutting willy-nilly.

I guess that's the biggest piece of advice I can give - make sure you're completely comfortable with your surgeon, your life is in his/her hands, literally. Do not go under the knife until every one of your fears have been allayed and your questions answered. This is a big scary, but trusting in the person cutting on you is a big step in making it all easier.

Hi that too was my biggest concern prior to surgery and the statistics my surgeon gave me were 4% will require an iliostomy usually due to a leak post surgery and of thoses the majority will be reversible so really really try not to worry about it chances are if the damage is limited to 1 section and the rest is healthy bowel you'll be fine. I've heard of people having half of there small intestine removed without needing a bag. I had 1 foot removed and no bag and no complications either. All the best and try not to worry but as Silvermander suggests make sure your comfortable with your surgeon if you have the choice. I was lucky enough to have elective surgery and therefore it was a more controlled environment and had a great surgeon.

Generally you will only have a stoma under these circumstances it you have extensive inflammation. If that was the case they would only remove what was absolutely necessary and form a temporary stoma, for about 3-6 months, so the still inflamed bowel has the opportunity to heal before they connect you back up again.

If on the other hand it is only the 15cm involved then you should have that removed without the need for an ileostomy. My son recently had a resection of 15cm of troublesome terminal ileum and he did not require a stoma. The only thing to bear in mind is that they will take a little more bowel out than just the affected area to ensure the margins are free of disease.
