Ostomy bag leaks all the time

My mom has a ostomy and her bag leaks all the time. She has tried all different types/companys but still leakage. I was wondering if someone out there has found a really good brand or adhesive to stop these accidents. She is 87 and not very active, so moving around isn't the problem. She gets a good seal in the beginning but as the day goes on, leakage starts. I'd love your input! Thanks!
 
This is a tough one because there are many factors.
Does she have an Ileostomy or Colostomy?
Does she wear her bag under her pants? Is the bag cut off by the belt line?
Is her output liquid or thicker?
Does she use a barrier ring like Adapt or Eakin?

What is she using currently? Brand and one piece or two?
Does she use any kind of Ostomy belt? Like the thin Hollister belt?
Who changes her flange? Is the skin around the stoma perfectly dry? I mean perfect. Using stoma powder helps. When you rub that around, you'll know if it's not totally dry.
Also dry the area using paper towels or tissues.

I didn't know some of the answers, so I called my dad (who takes care of my mom and her bag). She has a colostomy. She wears it under her pants. The bag is low, her stoma is equal with her belly button. Her output is like pudding. She used an Eakin barrier ring and it's a once piece. The pouch brand name is Coloplast - Assura. She does not use a belt. My dad said he makes sure the skin is very dry, but he has never used a powder. He said they used powder in the hospital and the whole thing came off in one hour and had a huge mess. Since my mom is 87, her skin is wrinkled, so they make sure to pull her skin tight as they put it on.
So any thoughts about her problem?
 
I have a colostomy as well, and I use the Coloplast SensSura line. I've never had a problem with leakage, and I don't even use a paste or anything. I also use a 2 piece system and bag liners from colomajic (they're fantastic! when they're full, you just pull it out of your bag and flush it). I think the problem with pulling the skin tight, when it contracts back it might form pockets where leakage could occur. Maybe you should call the appliance manufacturers and see what they say and get them send out some free samples so she can see what's going to work for her.

Hopfully Cindy is right, but I wonder what you could do about that. If you don't pull the skin tight, then you would be putting the flange on wrinkled skin anyway. (Has he ever tried that?)
It sounds like your dad is doing the right things. Just check if her pants are cutting off the lower part of the bag. If so, the ouput could get stuck up top and eventually undermine the eakin seal.

Maybe an ostomy nurse could help. If you don't have one, they have a web site to find someone local. Let me know if you need the site.
