Peri Anal Abscess and fistulas and setons

Hey there,
So In feb I had surgery for a horseshoe shaped peri anal abscess which were cause by a fistual about 3 inches up from my rectum, I had a seton ( thread kind) put in. Its a horrid thing to have to go through and by far my worst crohns experience. Well 3.5 months later and I have another abscess same area..I am having more surgery on June 14th.. it will end with another seton. I also have a pending surgery for a huge stricture ( 7 inches and the size of a drinking straw) in my transverse colon. I also have another large fistula in my small bowel they have to take care of. I have been living with this stricture for over 2 years and I live on smoothies and LOW LOW fiber food.. I have no energy and I am constantly running fevers. I am looking at august or september for the stricture surgery ( I need to heal from the seton/abscess) I am in a study for a new drug that is helping me and if I come out of the stricture surgery without a bag I can stay in it.
I have been on every drug out there... and they have failed.. I am now in a study.. which is kinda working. My next drug is Tresabray ( spelling is wrong) and I do not want to go to this drug... so I hope that I can stay where I am or find a new treatment.. I just rambled on a ton.. sorry about that. My question is will these darn abscess's heal and do setons stay forever.. and is it painful to get them removed?

Some people on this forum have had their setons for years.

I also had surgery in February on three fistulas, one complex, two not so complex, and my setons are suppose to come out in a month, but I also don't have a confirmed Crohn's diagnosis, so far they are calling my condition "Complex Perianal disease".The healing process was still really slow, and only really got better after a three week course of cirpo and flagyl.
 
Mine took 7 years to heal, had loads of MRI's EUA's and setons and I had three setons in for a long time in a complex horseshoe fistula.

I realise I am one of the lucky ones but you should not give up hope and try to stay positive.

My colorectal surgeon said that for the best chance for them to heal the rest of your crohns symtoms need to be in remission.
 
I always get a kick out of the names of these conditions- they all basically mean we have no clue what's really causing it (these names are simply descriptions). The first doctor whoever diagnosed me told me I had "nonspecific ulcerative colitis". Like there is somehow a specific form he didn't want to confuse me with. :tongue: That's the the nature of IBD, and why so tricky to correctly diagnose. Treatment and diagnosis seem more of an art form than a science, but finding the right combinations sure do help. ;)

I have two peri rectal fistulas. The one on my right side was a large(fist size) abscess that ruptured last march, i got a seton put in last june, so have had it for just over year now. Its not a thread, like a plastic tube. Can be very irritating. That fistual is a complex horse shoe shaped as well and they dont want to do surger to close it as the surgeons all day that it means cutting too many sphcincter muscles and that would be bad...The fistula on my left side has no drain it. It drains occasionnally on its own. Yesrerday it was quite sore and draining. This morning seems better. It does that once in a while. I have been on countless antibiotics and they dont seem to work anymore so i have stopped going to emerg to get more. Whats the point? I soak in countless sitz baths when they are active and then try an have a quick soak once a day when they arent. Right now i have been dealing with alot of diahreea and therefore my right side fistula seems to be aching nonstop......heat works the best i find. I have no idea how long i will hve this seton for. There is talk of when i see the GI that medication can be used to try and close them up. I am personally getting sick and tired of having my butt cheek ache all the time. Which isnt fair considering i probably went weeks without feeling a thing...now its just seems to be a bad couple of weeks for it. Seems unfair, my tummy feels great and by butt hurts. When my tummy was hurting, my butt felt good. Cant seem to win.

I am hoping one day to get this seton out. And i often wonder how much it will hurt.....cause getting it in was no picnic. But then i try and remember it only hurt for a few days after surgery and surely after everthing else i have gone through i can handle that again.......i think.....

i just need some good pain meds!!! :)

I had a perianal abscess back in 2006ish, I have previously had so many ops/issues I tend to forget the exact dates. My one went deep and was considerably large. It leaked a non-offensive fluid and I eventually had to have a vac-pump inserted. The sponge soaked up the fluid and subsequently, over a period of time, began to heal the abscess from the inside. I had the pump for approx 6 weeks and had it changed, under anaesthetic, fortnightly. It did eventually heal completely dry and I have had no repeat.

Admittedly that was a few years ago and it was here in the UK, so I am not sure how much it differs from the experience you will have. They do heal and it may take some time, but I tend to think that if it takes that little bit longer then, at least you know it is healing correctly. Whatever they do, I'm sure it will work out for you.

Can I ask a personal question to what your symptoms are?

I have been getting belly pains/cramping after eating (big or small as a bite) , direhha , nausea and burning when I have a BM like if I ate chili peppers.
 
My daughter had two Setons placed in Jan 2013. They have removed one that has healed (superficial), and the other (through anal sphincter) remains. The ticket for her is the biologic (Remicade) which is healing the tract. It makes her very tired after tx, but better than reabscessing. The surgeons say do not hurry taking these out. Always a chance of reabscess. Yes, worry about possible what ifs with these drugs, but we can't have a life without them. I'm afraid my other daughter (also with Crohns) is in the category with those of you who are failing most all drugs. She has had Crohns for 15 yrs with recent bowel resection/ileostomy. Now on Cimzia/Imuran . Trying reversal next week. Will see if this combo of drugs are working or not. Wish she were in a study already.
