When you find something wrong in your medical file, what do you do?

I reviewed my GI files and found inaccuracies. These followed to other specialists in the clinic since files are shared electronically. I thought this was a good idea. Each doc would know about my IBD if I stay within this clinic. Now that I see some items were wildly incorrect, I think this is a poor idea. For instance, records say there was a fistula to kidney (no, this is wrong) and surgical information is wrong.

What do you recommend for correcting medical files?

Is there a Patient Advocate or Patient Relations you could talk to? I had a similar issue and the Patient Relations person was able to help me. I was able to get an addendum to my record. You could also try talking to someone in charge in the clinic. Or, talking to someone in Medical Records.

I'd speak to the doctor who wrote down the wrong information. My old GP would sometimes write down the wrong thing cause he's always in a hurry and I had him correct it on the spot during my last appointment with him. Its not the system that's bad (its great and a lot better than the old system especially if you're shopping for a good doctor and they loose your file or its in the wrong office) its the doctors who don't pay attention. Same thing happens on paper documents so if you see false information, have it corrected by your doctor.
 
Thank you for your replies. I think I will have to request corrections in writing. We have moved to a different state, which is why I requested the medical records. I was surprised to see mistakes when I read through them. I was also surprised to see a couple negative subjective comments. I would not return to this particular doctor or his practice if I move back to that area.
