Hello.. Has anyone had this test done? My 15 year old son was diagnosed with Crohn's little over a year ago due to no growth, no puberty and constipation. He was started on Remicade infusions every 8 weeks until 2 months ago.. He was doing well in terms of growth and puberty has started but his remicade level was at 2.6 with no antibodies. at that time i noticed what looked like a psoriatic patch and thought it would go away.. doc moved his infusions to every 4 weeks ( nov, 2, november 30, december 28). Last couple weeks, I noticed he has 4 big patches on his legs now and seem to be getting worse. He's scheduled for another infusion at end of January and doctor wants to order this predictr test.. has anyone had this done? we are seeing dermatologist in 2 weeks.. doc said that he wants to hold on to remicade as long as possible and might add methotrexate.. I don't know how I feel about having him on 2 immunosuppressants.. anyone been in this situation? his initial calpoprectin before diagnosis was about 4000, then went to 88 and now back up to 1300...

So crohns tends to buddy up with skin conditions such as psoriasis and juvenile arthritis.
The genetics overlap
Biologics can unmask the condition or it might have popped up anyways.
Some find if they stop the biologic and switch to a different one then
33% see the psoriasis go away
33% see the psoriasis stay the same and
33% see the psoriasis get worse

My kiddo was on remicade at age 8
Lots of different skin rashes and eventually after 8 months he had anaphylaxis (x2) to remicade
Second time they slowed infusion gave steriods etc … did not help
He switch to humira and rashes left
But he developed juvenile arthritis and sweets syndrome
Rheumatology added methotrexate at the age of 10 to humira
He is now 21
He takes Stelara and methotrexate
Plus second biologic for his auto inflammatory condition (sweets syndrome )
He is enjoying life in college (4.0 deans list etc..)
So while getting psoriasis is less than ideal most recommend if the crohns is under control see dermatologist to treat the psoriasis.

I appreciate your response. Glad your son is doing well. Hopefully dermatologist can figure this out and manage it topically but have to anxiously wait until then :(. so many unknowns with this rash and upcoming blood work. Can't never get comfortable with this disease, something alway pops up :(
 
Hi I understand your concern and I felt the same, however, my son has been on Remicade for over 6 years and two years ago, Methotrexate was added to his regime. He also was on a 4 week schedule for the longest time as he needed a higher dosage, but through trial and error at the beginning of his treatment, eventually his levels reached a good level and he has been doing great. Your son's levels are low so getting them to a better place (my son's doctor likes to see around 10) will make a difference for your son. Also, my son being on both Remicade and MTX, there has been no difference in his health in terms of getting colds/flus. In fact, he gets them less than the general population.
 
Thank you so much. it's comforting to hear. how old is your son? My doctor wants his level also at 10. I don't know if puberty has anything to do with it but heard that hormones can increase inflammation. Did your son have any skin issues from remicade? My son's weight and height has significantly improved but based on last colonoscopy Crohn's better but not controlled yet. will see what this predictr test will show us and what derm will tell us.. thank you for making me feel more at ease :)
