I had my first periannal abscess 11 years back when I was 24, at 2 o'clock position. It was drained and packed regularly and it healed successfully in little more than 2 weeks (well I would like to believe so)! and I did not have any problem for 11 years.
Subsequently, I moved to London from India in 2015 and I developed another periannal abscess at 10 0'clock position for which I underwent another drainage on 24th Aug, after failed attempts of curing it through antibiotics.
However, my surgeon noticed that I had something at my old 2 o'clock postion which looked like fistula and he had forewarned me that I might have to undergo another procedure.
As it happens in such cases, my 24th Aug surgery wound did not heal up completely even after 9 weeks and though an MRI on 15th Oct did not reveal anything, my surgeon found fistula tracts at both 10 0'clock and 2 o'clock positions.
On 6th Nov, he laid open a tract at 10 o'clock position, and put a Seton at 2 o'clock position.
A 16th Nov I had a follow up appointment with my surgeon where he was satisfied with the progress.
After 2 weeks of packing/digitating the 10 0'clock wound, it is now bleeeding lesser day by day, however, my 2 o'clock wound would often bleed.
There has not been any ointment or medicine prescribed whatsoever, apart from the first 7 days of antibiotic metronidazole and some constipation relief powder (Movicol).
After 4 months of this cat and mouse game, I am now frustrated and also worried whether I am moving in the right direction or not.
